Fyzioterapie u reverzní totální endoprotézy ramenního kloubu

Physiotherapy in total Arthroplasty

Abstract:

Cíl: Cílem práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u klientů po operaci reverzní totální endoprotézy ramenního kloubu. Metodika: V metodice je popsána kasuistika 57- ti letého muže s diagnozou omarthrosis l. dx. posttraumatica, kdy fyzioterapeutická intervence trvala 2,5 měsíce. V časné pooperační fyzioterapii a následné ambulantní péči byly využity neinvazivní metody - základní antropometrická a goniometrická vyšetření, vyšetření aspekcí a palpací, techniky manuální medicíny hodnotící aktuální stav měkkých tkání, ROM a orientačně svalovou sílu pletence ramenního dle Jandy a funkční pracovní rozsah hybnosti. Na základě výsledků vyšetření byl zvolen individuální fyzioterapeutický plán pro jednotlivá období. Výsledky: Výsledky průběžných vyšetření prokázaly vliv operačního zákroku a následné pooperační fyzioterapie k navrácení klienta k plnohodnotnému životnímu standardu, opětovnému zařazení do pracovního procesu a minimálně ke zlepšení hybnosti a zlepšení funkčního pracovního rozsahu hybnosti horní končetiny. Na podkladě výsledků jsem aplikovala průběžnou terapii polohování, respirační fyzioterapie, myofasciální techniky, PIR, PFS, PNF, DNS, cvičení dle Brüggerova konceptu, Spiraldynamik?, SMS tahy, cvičení s využitím overbalu. V průběhu terapie došlo u klienta ke zlepšení ROM, posílení svalové síly a funkčního pracovního rozsahu pletence ramenního a celé HK. Závěr: Využitím včasné intervence a komplexní fyzioterapeutické péče jsem u klienta s reverzní totální endoprotézou ramenního kloubu dosáhla zlepšení hybnosti, posílení svalové síly, funkčního pracovního rozsahu a to vedlo u klienta ke zlepšení životního standartu, navrácení do pracovního procesu a společenského života.

Abstract:

The objective of the thesis is to contribute to solving the issue of selecting ideal physiotherapeutic procedures, concepts and methods in clients after the surgery of reverse total shoulder arthroplasty. In the methodology of timely post-surgery physiotherapy and subsequent outpatient treatment, non-invasive methods were applied - basic anthropometric and goniometric examinations, examination of physical signs and palpations, the technique of manual medicine evaluating the current condition of soft tissues, ROM and indicatively the muscular strength of shoulder girdle according to Janda, and the functional working range of motion. Based on the results of the examination, an individual physiotherapeutic plan for individual periods was selected. The results of continuous examinations demonstrated the effect of the surgery and of the following post-surgery physiotherapy on getting the client back to his standard of living, re-engagement in the work process, and at least improvement of motion as well as of the functional working range of the upper limb's motion. Upon the results I applied continuous therapy - positioning, respiratory physiotherapy, myofascial techniques, PIR, PFS, PNF, DNS, exercising by the Brügger's concept, spiral dynamics, SMS tensions, exercising with overball and Theraband. During the therapy, the client felt improved ROM, strengthening of muscular strength and of the functional working range of the shoulder girdle, as well as of the entire upper extremity. By using timely intervention and complex physiotherapeutic care in a client with reverse total shoulder arthroplasty, I managed to improve the motion range, support the muscular strength and the functional working range, and all that resulted in better standard of living in the client, returning to the work process and social life.
