Validation of methods of indirect comparison with different availability of data sources

Abstract:

This thesis deals with methods for achieving comparability of the baseline characteristics of own data with the data of a selected study or their summary results. Selected study usually differs from the author's in many parameters such as selection criteria, clinical indicators examined or patient characteristics themselves. In this case, it is not possible to determine whether the resulting difference obtained is due to the treatment characteristics or the cross-trial differences, which leads to biased estimation of treatment effect. Adjustment to the baseline characteristics allows to compare treatment outcomes across balanced trial populations without bias. This work introduces two types of methods of adjustment. First, in the availability of full data and second, methods adjusting individual data from own study to aggregate descriptive statistics of the selected study. The aim of this work is to introduce the reader to the issue of indirect comparisons and then to apply selected methods to real data of patients with acute heart failure syndrome coming from the Czech AHEAD registry. The practical part is focused on the ability of individual algorithms to adjust selected prognostic factors between groups in terms of average values and standard deviations, also evaluates the reduction of data set caused by adjustment and focuses on the very result of comparing two groups of hospitals with respect to the observed endpoint. Individual methods are tested at different settings and different numbers of adjusted prognostic factors. The best adjustment results were achieved with the application of the MAIC method, which was the only one able to balance out the not only averages but also the standard deviations. The resulting comparison of the clinical indicator was similar in a larger number of adjusted variables, whereas the difference between the adjusted groups was statistically insignificant for all methods. However, with a less variables, all the methods except the MAIC, which adjust for standard deviation, confirmed the difference.

Abstract:

Tato rigorózní práce se zabývá metodami pro dosažení srovnatelnosti bazálních charakteristik vlastních dat s daty vybrané studie nebo jejich sumárními výsledky. Srovnávány konkurenční studie se obvykle liší od autorské v mnoha parametrech jako jsou selekční kritéria, zkoumané klinické ukazatele, nebo samotné charakteristiky pacientů. V takovém případě nelze určit, zda výsledný získaný rozdíl je dán vlastnostmi léčby nebo vstupními parametry. Adjustací na bazální charakteristiky pacientů lze dosáhnout srovnatelnost datových souborů a vyhnout se zkreslení výsledků. V této práci budou představeny dva typy adjustačních metod. Jednak metody adjustace při dostupnosti plných dat a také metody adjustujíce vlastní individuální data na agregovaně popisně statistiky zvolené studie. Cílem práce je uvést čtenáře do problematiky nepřímých srovnání a následně aplikovat vybrané metody na reálná data pacientů se syndromem akutního srdečního selhání pocházejících z českého registru AHEAD. Praktická část je zaměřena na schopnosti jednotlivých algoritmů adjustovat vybrané prognostické faktory mezi skupinami z pohledu průměrných hodnot i směrodatných odchylek, také hodnotí redukci datového souboru způsobenou adjustací a zaměřuje se i na samotný výsledek porovnání dvou skupin nemocnic s ohledem na sledovaný klinický jev. Jednotlivé metody jsou testovány při různých nastaveních a různém počtu adjustovaných prognostických faktorů. Nejlepší výsledky adjustace byly dosaženy při aplikaci metody MAIC, která jako jediná dokázala vyvážit mimo průměrů i hodnoty směrodatné odchylky. Výsledné srovnání klinického ukazatele dopadlo při větším počtu adjustovaných proměnných obdobně, přičemž rozdíl mezi skupinami po úpravách souboru byl statisticky nevýznamný při všech metodách. Při menším počtu však všechny metody, s výjimkou MAIC upravující směrodatnou odchylku, rozdíl potvrdili.
