Compress Therapy Effect after the Sclerotherapy of Varicose Veins of Lower Extremities

Abstract:

Úvod: Aplikace kompresních pomůcek, bandáží nebo kompresních punčoch po intervenční léčbě je jedním ze základních postupů ve flebologii. Jejich použití je vždy doporučováno jako rutinní postup po intervenční léčbě křečových žil, včetně skleroterapie, v platných guidelines. Existuje však jen malý počet studií, které by měly jako svůj hlavní cíl vytvořit doporučení jejich použití po intervenční léčbě na základě vědecky podložených důkazů. Cíl: Cílem práce je prokázat vliv kompresní terapie na riziko vývoje sklerotrombu po provedené skleroterapii. Materiál a metodika: U 80 pacientů byla provedena kompresní terapie v krátkém nebo dlouhém režimu (u čtyř bylo použito nízkotažné obinadlo, u zbylých 76 kompresní punčocha). Jednotlivý režim byl přidělen každému pacientovi individuálně, na základě randomizačního klíče. Jednotlivé parametry jako je vznik sklerotrombu, riziko rekanalizace, typ skleroterapie, charakter provedení zákroku, forma sklerotizační látky (kapalina nebo pěna), a další byly zaznamenávána v souladu s protokolem studie. Výsledky: Ve skupině s krátkým režimem komprese (skupina A ? 1 týden po zákroku) se vytvořil sklerotromb ve 21 případech, u 19 pacientů se sklerotromb nevytvořil. Ve skupině B ? dlouhý režim (14 dnů po intervenci) se sklerotromb vytvořil v 17 případech a v 23 nebyl přítomen. Obě skupiny vykazovaly statisticky významný rozdíl ve smyslu redukce žilní bolesti po provedeném zákroku (p = 0,001) a ve smyslu redukce pocitu těžkých, unavených nohou (p = 0,011). Ke statistickému hodnocení byl použit ?2 test. Tyto výsledky nebyly ovlivněny typem žíly. Nebyl pozorován statisticky významný rozdíl mezi oběma režimy (p = 0,370, použit ?2 test) v parametru rizika vzniku sklerotrombu. U tohoto para-metru byl prokázán statisticky významný rozdíl pouze u teleangiectazií (p = 0,014, použit ?2 test). V případě rizika rekanalizace nebyl prokázán statisticky významný rozdíl mezi oběma režimy. Závěr: Krátkodobý režim komprese po skleroterapii je zcela dostačující. Aplikace komprese delší dobu po skleroterapii neovlivní riziko vzniku sklerotrombu a riziko rekanalizace sanované žíly. Pouze u teleangiektazií lze doporučit aplikaci komprese po delší dobu.

Abstract:

Introduction: The application of compression bandages or stockings after a patient's undergoing invasive treatment is one of the standard procedures in phlebology. In the current guidelines on the treatment of varicose veins, compression is recommended as a routine measure following varicose vein intervention including the sclerotherapy. But only few papers exist that study this topic with evidence based recommendations in their conclusions. Aim: the aim of the study is to determine the influence of the compression therapy to the risk of the formation of the sclerothromb (trapped blood) in the veins after sclerotherapy. Material and Methods: 80 patients were applied short or longer regimen of compression (bandages ? 4 patients and compression stockings ? remainig 76 patients). The regimen was assigned to one individual patient according to randomized key. The formation of the trapped blood, risk of recanalization, type of sclerotherapy, the intervention accomplishment, the form of sclerosing substance (liquid or foam) and other parameters were noticed according to the study protocol. Results: The group with shorter regimen (group A ? 1 week only after intervention) was associated with establishment of sclerothromb (trapped blood) in 21 cases and with no sclerothrom in 19 cases. In the group B ? longer regimen (14 days after intervention) was sclerothrom established in 17 cases and no sclerothromb was in 23 cases. The statistic sig-nificant difference (p = 0,001) was found in both groups comparing the venous pain before and after sclerotherapy. The statistic significant difference (p = 0,011) was found in both groups comparing the fatigue before and after sclerotherapy. The types of vein not influ-ence these results. No statistic significant difference (p=0, 370) was observed between both regimens using the ?2 test at the risk of forming sclerothromb. The statistic significant difference (p = 0,014, using the ?2 test) was found only in teleangiectasies, comparing both groups in parameter at the risk of sclerothromb forming. No statistic significant difference was observed between both regimens using the ?2 test at the risk of recanalization. Conclusions: The shorter regimen of compress therapy is sufficient after this intervention. The longer wearing of the compression does not influence the risk of forming a sclerothromb and decreased the risk of recanalization. Only in the case of teleangiectazies the longer regimen is recommended.
