Psychological Characteristics of Hyperkinetic Disorder: Interindividual Differences in Hyperkinetic Boys Aged 7-13 years

Abstract:

Hyperkinetické poruchy (HKP) patří k nejvýznamnějším a stále aktuálním tématům dětské klinické psychologie a psychiatrie. HKP jsou definovány v současné verzi Mezinárodní klasifikace nemocí (MKN−10) a jsou charakterizovány triádou jádrových příznaků, poruchou pozornosti, hyperaktivitou a impulzivitou. Podobnou, ale ne identickou diagnostickou kategorií je attention−deficit hyperactivity disorder (ADHD), definovaná v současné verzi Diagnostického a statistického manuálu mentálních poruch (DSM−IV). V MKN−10 i v DSM−IV jsou uvedeny subtypy těchto diagnostických kategorií, které jsou však založeny na rozdílném kritériu rozlišení. V této disertační práce shrnujeme základní teoretické poznatky o HKP, přičemž se podrobněji zaměřujeme na teoretické modely HKP/ADHD a na současné výzkumy subtypů HKP/ADHD. Náš výzkum byl inspirován studiemi srovnávajícími subtypy ADHD definované v DSM−IV a studiemi empiricky odvozujícími subtypy ADHD na základě měření psychologických charakteristik. Cílem naší studie je popsat psychologické charakteristiky související s jádrovými příznaky HKP a interindividuální rozdíly v klinické populaci chlapců s hyperkinetickou poruchou ve věku 7−13 let. Naším hlavním cílem je identifikovat chlapce s podobným profilem výkonu psychologických charakteristik. Použili jsme testy typu „tužka−papír“, počítačem administrované neuropsychologické testy a posuzovací dotazníky a škály. Výzkum popisuje výkon hyperkinetických chlapců vzhledem k normě. Srovnání subtypů HKP definovaných v MKN−10 potvrzuje validitu těchto subtypů a nachází rozdíly v některých dalších výkonových, behaviorálních a podrobnějších diagnostických charakteristikách. Na základě výkonu v testech „tužka−papír“ (v testu pozornosti a v testu impulzivity) a za použití shlukové analýzy jsme vyčlenili čtyři subtypy s rozdílným profilem výkonu. Tyto subtypy jsme srovnali z hlediska dalších výkonových a posuzovaných charakteristik. Zjistili jsme rozdíly pouze v některých výkonových charakteristikách, nikoliv však v behaviorálních a diagnostických charakteristikách. Naše práce prezentuje zjištění o subtypech HKP a demonstruje možnosti výzkumu subtypů HKP, které mohou být významné pro další zkoumání i praktickou aplikaci.

Abstract:

Hyperkinetic disorder (HKD) is one of the most important and still up−to−date issue of the child clinical psychology and psychiatry. HKD is defined in the current version of the International Statistical Classification of Diseases (ICD−10) and is characterized by a triad of core symptoms, namely inattention, hyperactivity, and impulsivity. A similar, but not identical, diagnostic category is Attention−deficit hyperactivity disorder (ADHD) defined in the current version of Diagnostic and Statistical Manual of Mental Disorders (DSM−IV). Both the ICD−10 and the DSM−IV define diagnostic category subtypes, however based on different distinction criteria. This doctoral thesis summarizes the basic theoretical knowledge about HKD, particulary focusing on theoretical models of HKD/ADHD and on present HKD/ADHD subtypes research. Our presented research is inspired with the studies comparing ADHD subtypes defined in DSM−IV and with the studies empiricaly deriving ADHD subtypes on the basis of psychological measures. The aim of the study is to describe psychological characteristics associated with HKD core symptoms and interindividual differences in clinical population of hyperkinetic boys aged 7−13 years. The main objective is to identify boys with similar psychologic performance profiles. Psychological methods as "pen−and−paper" performance tests and computer administrated neuropsychological tests, and assessment questionnaires and scales were used. The research describes the hyperkinetic boys performance compared to the normative data. The comparison of HKD subtypes as defined in the ICD−10 confirmed the subtype validity and founnd diferences in some performance and behavioral characteristics, and detailed diagnostic characteristics. Using "pen−and−paper" attention and impulsivity tests and cluster analysis were derived four subtypes with different performance profile. These subtypes were compared in other performance and assessment characteristics for clinical relevance. Differences in only some other performance characteristics, but not in behavioral and diagnostic characteristics were found. The study presents findings about HKD subtypes and demonstrates the HKD subtype research possibilities, which could be important for other research and pratical application.
