Abstract:

Úvod Horečka je u kriticky nemocných pacientů známka základního problému, který je třeba objevit a vhodně řešit. Horečka je definována jako teplota jádra >38,3 °C. Horečka je obvykle léčena zpočátku antipyretiky, když je odpověď organismu na léky omezená, nebo pokud je nezbytné snížit teplotu pacienta rychle, používá se zevní chlazení. Metodika Data získávala autorka z dokumentace pacientů a zaznamenávala do výzkumného protokolu. Tělesná teplota byla měřena sestrami pomocí digitálního teploměru v Městské nemocnici a pomocí skleněného kontaktního bezrtuťového teploměru ve Fakultní nemocnici Ostrava. Sběr dat se týkal zjištění věku a pohlaví respondentů. Autorka zjišťovala aktuální APACHE II v době přijetí pacienta na stanici. Dále zjišťovala, zda byl pacient sedován, zda mu byla podána antipyretika a kdy mu byla podána. Autorka zjišťovala, jak dlouho byl pacient chlazený, než došlo k normotermii a zda došlo k návratu horečky. Pokud došlo ke snížení horečky, za jak dlouho se horečka vrátila. Zároveň zaznamenávala horečku, při které bylo započato chlazení. Do studie byli zařazeni pacienti, kterým se horečka vrátila do 24 hodin. Chlazení pacienta bylo vždy započato dle ordinace lékaře a taktéž podání antipyretik bylo rozhodnutím lékaře. Pro statistické vyhodnocení byl použit ?2 test a Fischerův exaktní test. Pro testování průměrných hodnot věku a teploty dle druhů chlazení byl použit t-test pro dva výběry. Výsledky Při porovnávání dvou sledovaných skupin pacientů bylo zjištěno, že došlo k návratu horečky do 24 hodin po ukončení chlazení v 1. skupině u 80 % pacientů a ve 2. skupině u 73,33 % pacientů. Analýzou dat bylo zjištěno, že není rozdíl v návratu horečky mezi skupinou chlazenou přístrojem Blanketrol III a skupinou chlazenou gelovými chladivými polštářky. Závěr Horečka je u kriticky nemocných pacientů běžnou reakcí na sepsi. Horečka se objevuje buď působením exogenního nebo endogenního pyrogenu na syntézu prostaglandinu E2 v preoptickém vláknu. Febrilní reakce je dobře zachována v celém živočišném světě, a experimentální důkazy nasvědčují tomu, že tato může být příznivou odpovědí na infekci. Horečka je ale u kritických pacientů léčena antipyretiky, aniž by existovaly dostatečné údaje, které by takovou praxi podpořily. Hlavním cílem práce bylo zjistit, zda při použití přístroje Blanketrol III dochází k opětovnému návratu horečky v porovnání s chlazením chladivými gelovými polštářky. Tento cíl tohoto výzkumného šetření byl splněn. Autorka zjistila, že k návratu horečky došlo v obou skupinách v přibližně stejném poměru. Můžeme tvrdit, že není rozdílu mezi tím, zda pacienty chladíme přístrojem Blanketrol III a přikládáme přikrývku s cirkulující vodou na celou plochu těla, nebo, zda-li přikládáme chladivé ledové polštářky pouze na třísla, do axily a k hlavě.

Abstract:

Introduction Fever is for critically ill pacients sign of a basic problem that needs to be discovered and cured properly. Fever is defined as a core temperature higher than 38,3 °C. Fever is usually cured with antipyretics at first, if the response to medicaments is limited or we need to lower pacient?s temperature quickly we use external cooling. Method Data are collected from documentation of pacienst and are put into research protocol. Body temperature was measured by nurses with digital thermometer in City hospital and with glass mercury-free contact thermometer in Faculty hospital Ostrava. Data collection focused on the age and sex of respondents. The author was searching for current APACHE II at the begginig of staying in the department. The author examined wether the patient was sedated if and when they were given antipyretics. The author researched how long was the patient cooled before normothermia and whether the fever returned. If the temperature was decreased and after what time did the fever return. The author also noted the fever which was the cooling started with. Pacients whose fever repeated in 24 hours were put into the research. The cooling always started according to the doctor?s office and giving the antipyretics was also doctor?s decision. For statistical analysis was used test ?2 and Fisher?s exact test ( in case of lack of number). For testing average values of age and temperatures according to the kind of cooling t-test for two selections was used. Result The research of two groups resulted that 80% of patients in the 1st group and 73,33% of patients in the 2nd group had the fever in 24 hours again. Analysis discovered there is no difference in return of fever between group cooled with Blanketrol III and group cooled with cooling gel pads. Conclusion Fever is for crtically ill patients common reaction to sepsis. Fever is caused by effection of exogenous or endogenous pyrogen on prostaglandin synthesis E2 in preoptic fiber. Febrile reaction remained well in the whole animal world and experiments show that it can be fair response of infection. However fever in case of critically ill patients is cured by antipyretics without any data that would support this practice. The main aim of theses was to discover whether the fever returns after using Blanketrol III compared to cooling gel pads. This aim was acomplished. The author discovered that the return is in both cases very similar. We can say there is no difference in cooling patients with Blanketrol III with using a cover with circulating water and using the cooling gel pads placed on groin, axilla and on head.
