The effect of the shock wave therapy on the foot load in patients with heel spur

Abstract:

Diplomová práce se zabývá problematikou patní ostruhy a jejím klinickým dopadem na tlakové zatížení chodidla. V této práci je pro ošetření patní ostruhy využita terapie rázovou vlnou. Cílem práce bylo zjistit, jaký vliv má terapie rázovou vlnou na tlakové zatížení chodidla v průběhu bipedálního stoje a stojné fáze chůze. Studie se zúčastnilo 12 pacientek (věk 48,3 ? 23,7 let; hmotnost 89,6 ? 39,4 kg; výška 166,5 ?15,5 cm, BMI 32,1?10,1). U všech pacientek byla diagnostikována plantární ostruha patní kosti (calcar calcanei) a lékařem indikována léčba rázovou vlnou. Všechny pacientky podstoupily 4 aplikace s týdenním odstupem. Pro změření tlakového zatížení chodidla bylo využito zařízení, které se skládá z pedobarografické plošiny freeMed (Sensor Medica, Itálie) a softwaru freeStep. Sběr dat proběhl dvakrát - před aplikací a 8 týdnů po poslední aplikaci rázové vlny. Měření probíhalo ve 2 fázích - v klidném stoji na obou dolních končetinách a při chůzi. Po terapii rázovou vlnou došlo k významným změnám zatížení chodidla u nohy s patní ostruhou. Díky redukci bolesti se ve stoji a při chůzi podstatně více zatížila postižená dolní končetina. Prokazatelné zvýšení míry tlakového zatížení nastalo také v oblasti patní ostruhy, tj. zadonoží a pata. Z výsledků práce vyplývá, že terapie rázovou vlnou byla hodnotným terapeutickým nástrojem pro léčbu patní ostruhy u sledované skupiny pacientek.

Abstract:

The thesis deals with the issue of the heel spur and its clinical impact on the compressive load of the foot. In this thesis, shock wave therapy is used for the treatment of heel spurs. The aim of this thesis was to investigate the effect of shock wave therapy on the compressive loading of the foot during the bipedal standing and stance phase of gait. Twelve patients (age 48.3 ? 23.7 years; weight 89.6 ? 39.4 kg; height 166.5 ? 15.5 cm; BMI 32.1 ? 10.1) participated in the study. In all patients, plantar calcaneal spur (calcar calcanei) was diagnosed and shock wave treatment was indicated by the physician. All patients underwent 4 applications one week apart. A device consisting of a freeMed pedobarographic platform (Sensor Medica, Italy) and freeStep software was used to measure the compressive load of the foot. Data collection was performed twice, before and 8 weeks after the last shock wave application. The measurements were performed in 2 phases - standing still on both lower limbs and during walking. There were significant changes in the foot loading in the heel spur foot after shock wave therapy. Due to pain reduction, the affected lower limb was significantly more loaded in standing and walking. A demonstrable increase in the rate of compressive loading also occurred in the heel spur area, i.e. the hindfoot and heel. The results of the study suggest that shock wave therapy was a valuable therapeutic tool for the treatment of heel spurs in the group of patients.
