Abstract:

Diplomová práce se zabývá komplexní problematikou Ca pankreatu, psychologií nemoci a autoplastickým obrazem nemoci. Dále popisuje holistické pojetí ošetřovatelství, kdy úlohou sestry je uspokojovat potřeby nemocného na všech úrovních ošetřovatelské péče. Nemocní s Ca pankreatu mají nepříznivou prognózu a jsou často hospitalizováni, proto úloha sestry je pro ně nepostradatelná. Dále teoretická část představuje statistické údaje z Národního onkologického registru a z 1. interní kliniky Fakultní nemocnice Plzeň, které upozorňují na incidenci Ca pankreatu, ale i na jeho mortalitu. Výzkumná část bude představovat získané informace z oblasti potřeb a přání hospitalizovaných pacientů, subjektivního významu hloubkového rozhovoru všeobecné sestry s pacientem a přínosu hloubkových rozhovorů pro všeobecnou sestru. Naši pacienti vnímají toto onemocnění jako velkou zátěž, která jim postupně ubírá nejen sílu ale i chuť do života. Mezi jejich přání pak patří jíst, netrpět bolestí, zůstat co nejdéle soběstačný a v kruhu svých blízkých. Z výzkumného šetření dále vyplývá, že nemocní strádají nejvíce v oblasti fyziologických potřeb a potřeb bezpečí a jistoty. Komunikace ve vztahu sestra - nemocný má velký význam, pacientům může dopomoci ke zlepšení jejich psychického stavu. Během našich rozhovorů jsme si uvědomily, že nepůsobíme jen my na nemocné, ale i oni na nás. Ovlivňují nás zejména v péči o ně, nevyžadují nic víc než jen pomoc se základními potřebami a vlídné chování. Jediné, v čem takto nemocným můžeme pomoci, je zajistit jim takovou péči a pomoc, kterou si oni přejí a kterou potřebují k tomu, aby zbytek jejich života byl kvalitní.

Abstract:

The diploma thesis deals with complex problems of pancreas Ca, psychology of the disease and autoplastic picture of the disease. It also describes the holistic concept of nursing, where the role of the nurse is to satisfy the needs of the patient at all levels of nursing care. Pancreatic Ca patients have an unfavorable prognosis and are often hospitalized, so the role of a nurse is indispensable to them. Furthermore, the theoretical part presents statistical data from the National Cancer Registry and from the 1st Internal Clinic of the University Hospital in Pilsen, which draws attention to the incidence of pancreatic Ca, but also to its mortality. The research part will present the acquired information from the area of needs and wishes of hospitalized patients, the subjective significance of in-depth interview of the nurse with the patient and the contribution of in-depth interviews for the nurse. Our patients perceive this disease as a great burden, which not only reduces their strength but also their desire for life. Their wishes include eating, not suffering from pain, staying self-sufficient for as long as possible and in the circle of their loved ones. The research also suggests that patients suffer most from physiological needs and safety and security needs. Nurse-patient communication is of great importance, helping patients to improve their mental state. During our conversations, we realized that we were not just acting on the sick but also on us. They affect us in particular in the care of them, they require nothing more than just help with basic needs and kind behavior. The only thing we can do to help the sick is to provide them with the care and help they desire and the need for the rest of their lives to be good.
