Intervention of autotherapy by patients with rheumatoid arthritis

Abstract:

Bakalářská práce se zabývá problematikou revmatoidní artritidy a její autoterapií. Revmatoidní artritida je chronické zánětlivé autoimunitní onemocnění postihující synoviální výstelku kloubů, burz a šlach s výskytem mnohých extraartikulárních příznaků. Cílem práce je vyhledat adekvátní možnosti fyzioterapie a navrhnout cvičební jednotku na jejímž základě vznikne edukační materiál pro autoterapii u pacientů s revmatoidní artritidou. V teoretické části jsem se zabývala anatomií kloubu, pojmem revmatologie a definováním revmatoidní artritidy. Současným stavem, etiopatogenezí, diagnostikou, klinickým obrazem, prognózou a konečně léčbou a terapií tohoto onemocnění. Do výzkumné části jsem zahrnula cíle práce, výzkumné otázky, metodiku a výsledky. Ke zpracování dané problematiky bylo zvoleno kvalitativní výzkumné šetření, za pomoci kazuistik s pěti pacientkami, u nichž bylo provedeno vstupní a výstupní vyšetření, které obsahovalo odběr osobních dat a anamnézy, kineziologický rozbor, Barthel Index Test a Four Step Square Test. Součástí vstupního vyšetření byl navíc podpis Informovaného souhlasu, plantogram a fotodokumentace rukou a nohou. Na základě vyšetření byl stanoven krátkodobý a dlouhodobý rehabilitační plán. Terapie byla u všech pacientek účinná. Pravidelnou intervenovanou autoterapií došlo ke zmírnění bolesti a ztuhlosti kloubů, ke zvýšení hybnosti především páteře a horních končetin a dále ke zkvalitnění funkce horních končetin a funkce nohy. Výsledky výzkumu poukazují na to, že metody fyzioterapie byly zvoleny adekvátně, jelikož efekt terapie je pozitivní. Tato práce a na jejím základě vzniklý edukační materiál může posloužit samotným pacientům s revmatoidní artritidou, ale i pacientům s jinými typy onemocnění pohybového aparátu. Dále pak fyzioterapeutům v praxi při fyzioterapii individuální nebo skupinové nebo ostatním jedincům při pohybové péči o jejich klouby a pohybový aparát.

Abstract:

The bachelor thesis deals with the problematics of rheumatoid arthritis and its autotherapy. The rheumatoid arthritis is a chronic inflammatory autoimmune disease affecting synovial lining of the joints, bursas, and tendons with many extra-articular symptoms. The target of the thesis is to find adequate options of physiotherapy and to suggest exercise unit which will create the basis for an educational material for the autotherapy of the patients with the rheumatoid arthritis. In the theoretical part, I have focused on the anatomy of the joint, the concept of rheumatology and the definition of the rheumatoid arthritis. The current condition, the etiopathogenesis, the diagnostics, the clinical picture, the prognosis, and finally the treatment and the therapy of this disease. Into the research part, I have included the thesis's targets, the research questions, the methodics, and the results. I have chosen a qualitative research survey with the help of the casuistries of five patients, who went through the input and output examination including the collection of personal data and anamneses, the kinesiology analysis, the Barthel, Index Test, and the Four Step Square Test. The input examination contained, moreover, the signature of The Informed Consent, the footprint, and the photo-documentation of the arms and the legs. The hands and the feet rehabilitation plan was laid down based on the examination. The therapy was successful in the all patients' cases. The regular intervened autotherapy caused the pain and joint stiffness relief, the momentum improvement mainly of the spine and upper limbs, and then the improvement of the arms and the lower limbs. The research results point out that the methods of the physiotherapy were adequately chosen because the therapy has the positive effect. This thesis and the educative material created on its basis can help patients themselves with the rheumatoid arthritis as well as the patients with other types of musculoskeletal-system diseases. It could then help the physiotherapists in practise with individual or group physiotherapy or the other persons with physical care of joints and musculoskeletal system.
