Connection between status of oral cavity and origin of pneumonia in elderly patients with decreased self-sufficiency

Abstract:

Bakalářská práce se věnuje tématu stavu dutiny ústní jako jednomu z rizikových faktorů vzniku nozokomiální pneumonie u seniorů se sníženou soběstačností. Cílem práce je vytvořit přehled poznatků na toto téma a přiblížení problematiky česky hovořícím, protože většina zdrojů k tomuto tématu je publikována v anglickém jazyce. Práce je rozdělena na teoretickou a praktickou část. Teoretická část se věnuje popisu problematiky dle uvedených odborných zdrojů. Praktická část se skládá ze dvou celků. První je anonymní dotazníkový průzkum mezi personálem zdravotnických zařízení Fakultní nemocnice Brno, Fakultní nemocnice u sv. Anny v Brně a Domova sv. Alžběty, Žernůvka. Hlavním cílem bylo zjistit informovanost všeobecných sester o problematice péče o dutinu ústní a jejich vztah k jejímu provádění. Druhá polovina praktické části předkládá výsledky výzkumu mapujícího mikrobiologické osídlení dutiny ústní pacientů během hospitalizace. Provedeným výzkumem bylo potvrzeno, že pacienti hospitalizovaní ve zdravotnických zařízeních jsou ohroženi kolonizací dutiny ústní respiračními patogeny a že celková úroveň hygieny ovlivňuje množství vzorků pozitivních na alespoň jeden respirační patogen. Přestože nebyl nalezen výrazný rozdíl v kvantitě kolonizace respiračními patogeny při užití chemických nebo mechanických pomůcek péče o dutinu ústní, výsledky se lišily druhy respiračních patogenů, které byly kultivovány.

Abstract:

This bachelor thesis deals with the state of the oral cavity as one of the risk factors for development of nosocomial pneumonia in the elderly patients with decreased self-sufficiency. The aim of this work is to create an overview of knowledge on this topic and present the issue to Czech-speaking readers, because most of the resources on this topic are published in English. The work is divided into theoretical and practical part. The theoretical section describes the issues according to listed reference resources. The practical section consists of two parts. The first is an anonymous survey among staff of FN Brno, FN u sv. Anny v Brně and Domov sv. Alžběty, Žernůvka. The main goal was to determine their awareness of the oral care and their relationship to its performing. The second half of the practical section presents the results of research investigating the microbiological colonization of the oral cavity of patients during hospitalization. It was confirmed that the hospitalized patients are at risk of colonization of the oral cavity by respiratory pathogens and that the level of oral hygiene affects the amount of bacteriological smears that are positive for at least one respiratory pathogen. However we found no significant difference in the quantity of samples positive for respiratory pathogens when patients used chemical or mechanical oral hygiene aids, there were different respiratory pathogens cultivated by these two groups of patients.
