The architecture of the descendent system of the auditory pathway and its possible role in tinnitus pathogenesis

Abstract:

Populace 131 pacientů byla vyšetřována z hlediska manifestace tinnitu. Při vyšetření bylo měřeno skóre emocionální lability, odpověďi BAEP a MLR a byla provedena analýza 9 genetických polymorfismů receptorů descendentního systému sluchové dráhy: Taq I polymorfismu DRD2 receptoru, čtyř polymorfismů podjednotky GRIN2A a 2664 C/T polymorfismus podjednotky GRIN2B NMDA receptoru, dále byla stanovena délka CA repetitivní sekvence pro GABRß3 podjednotku GABA-A receptoru, dále 1298 A/T polymorfismus cholinergního M2 receptortu a délka repetitivní sekvence v promotoru protodynorfinového genu. Byly nalezeny následující vztahy k manifestaci tinnitu: poměr amplitud vlny V/III u mení BAEP koreloval s manifestací tinnitu (p<0,001) a amplituda vlny PA u odpovědi MLR rovnž korelovala s tinnitus skóre (p<0,001) Manifestace tinnitu rovněž závisela na genotypu CA-repetitivní sekvence pro GABRß3 podjednotku GABA-A receptoru (p=0,002), a to v populaci pacientů se symptomatologií kratší než 9 měsíců a průměrným sluchovým prahem horším než 25 dB. U pacientů, kde byla anamnéza tinnitu delší než 5 let byla nalezena silná korelace se skóre emocionální lability (p=0,013). Tyto výsledky integrují některé teoretické modely patogeneze tinnitu, které byly dodnes publikovány.

Abstract:

A population of 131 patients was examined on the manifestation of tinnitus. During the examination, the score of emotional instability was measured, the BAEP and MLR measurings were taken, and the analysis of nine genetic polymorphisms was carried out Taq I polymorphism of the DRD2 receptor, four polymorphisms of the gene for the sub-unit GRIN2a, and the 2664 C/T polymorphism of the gene for GRIN2b sub-unit of the NMDA receptor, CA repetitive sequence in the gene for GABRß3 subunit for GABA-A receptor, 1298 A/T polymorphism of the gene for cholinergic M2 receptor, and the repetitive sequence in the promotor of the protodynorphine gene. The following relations were discovered: the manifestation of tinnitus correlated with the ratio of the amplitudes of V/III waves (p<0,001), and with the amplitude of the PA wave of the MLR (p<0,001). The manifestation of tinnitus also depended on the genotype of the CA repetitive sequence of the gene for GABRß3 sub-unit (p=0,002), which was found in the sub-population of patients whose case history lasted less than nine months and with aver-age hearing threshold higher than 25 dB. In patients with tinnitus history exceeding five years, close correlation was found between tinnitus manifestation and their emotional lability score (p=0,013). These results integrate some models of tinnitus pathophysiology published to this day.
