The physiotherapy for injuries tendon apparatus hand.

Abstract:

Tato bakalářská práce obsahuje informace týkající se poranění šlachového aparátu ruky. Většinu informací pro vytvoření předkládané práce jsem získala ze zkušeností pracovníků v Ústavu chirurgie ruky a plastické chirurgie ve Vysoké nad Jizerou, čerpala jsem také z odborné literatury. Práce obsahuje tři nejdůležitější části: část teoretických poznatků, metodiku a diskuzi. V teoretické části jsem se věnovala anatomii ruky, klasifikaci poranění flexorových šlach a možnostem jejich následné léčby a rehabilitace. Následně jsem zde rozebrala klasifikaci extenzorových poranění šlach a také léčbu a rehabilitaci. Uvedla jsem několik rehabilitačních metod a některé z nich jsem využila v následující praktické části. V metodické části popisuji spolupráci s klientem. Klient je po úrazu flexorového aparátu ruky v oblasti palce a vyskytla se zde i dekomprese nervu medianu. Byla odebrána anamnéza a vyšetření klienta zaměřené především na horní končetinu a dle zhotoveného vyšetření zahájena rehabilitace. Pro klienta byla zhotovena speciální dynamická dlaha a byl poučen, jak s danou dlahou zacházet, jak dlouho a jakým způsobem na ní má cvičit, následně byl klient pozván na kontrolu po 5 týdnech a byl vytvořen nový rehabilitační plán. Klient byl edukován po fyzioterapeutické i ergoterapeutické stránce. Následně byl upozorněn na činnosti, kterých by se měl vyvarovat a které jsou pro něj vhodným tréninkem. V diskuzi porovnávám možnosti rehabilitačních metod a odůvodňuji můj výběr rehabilitace a její výsledek.

Abstract:

This thesis contains information related to tendon injuries of hand apparatus. Most of the information for this thesis has been obtained thanks to the experience of employees of the Department of Hand Surgery and Plastic Surgery in Vysoká nad Jizerou, while extensive amount of data was taken from related literature. The thesis is divided into three main sections: the theoretical part, methodology and discussion. Theoretical part is devoted to the anatomy of hand, hand flexor tendon injury classification and the possibilities of the treatment and rehabilitation to follow. Subsequently, the classification of the extensor tendon injury was analysed as well as the treatment and rehabilitation. Several rehabilitation methods were presented and some of them were used within the following practical part. In the methodological part the cooperation with the client is described in detail. The client has suffered an accident of flexor hand apparatus in the thumb area together with the decompression of the median nerve. The anamnesis has been prepared; the examination of the client focused primarily on the upper limb accomplished and based on the latter, the rehabilitation was introduced. The client was provided with a special dynamic splint, fabricated right for the patient, and the client was instructed how to manipulate it, how long is the client supposed to use it, etc. After that the client was invited to meet for a check-up in five weeks' time, when a new rehabilitation plan was set up. The client was instructed regarding physiotherapy and occupational therapy. Subsequently, the client was warned about the activities to avoid and instructed about the most suitable exercise. In the discussion the rehabilitation methods are compared, my choice of rehabilitation methods is justified and its outcome is commented on.
