Abstract:

ABSTRAKT Název: Otevřená zlomenina ruky s přetětím adduktoru palce Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta po otevřené zlomenině ruky v oblasti zápěstí a palce. Metodika: Ve své bakalářské práci pracuji s pacientkou, která při nehodě utrpěla otevřenou zlomeninu ruky s poraněním adduktoru palce. Po vstupním kineziologickém rozboru jsem vypracovala krátkodobý fyzioterapeutický plán, v jehož průběhu plnění jsem použila různé druhy konceptů a metod. Z těch nejčastěji používaných bych zmínila techniky měkkých tkání, protažení zkrácených struktur, posílení oslabených struktur, mobilizace zápěstí a drobných kloubů ruky, zvětšení rozsahu pohybu, zvětšení svalové síly, nácvik úchopů, míčkování, ježkování, proprioceptivní neuromuskulární facilitaci (PNF), rytmickou stabilizaci (RS), postizometrickou relaxaci (PIR) svalů předloktí a ruky. Výsledky: Díky pravidelné rehabilitaci došlo k výraznému zlepšení svalové síly, rozsahu pohybu v daných kloubech a jemné motoriky. Jizva a pokožka, kterou část rehabilitace zakrývala dlaha, se hojila s menšími komplikacemi (podráždění pokožky, vyrážka). Závěr: Podařilo se nám obnovit plné rozsahy pohybu v zápěstí a ukazováku. Rozsah pohybu v karpometakarpálním a metakarpofalanfovém kloubu palce se podařilo zvětšit a však ne do plného rozsahu. Při úchopech ruky je nynější rozsah pohybu palce plně postačující a vyhovující. Abstract:

SUMMARY Title: An open fracture of the hand with an overcut of the thumb adductor The aim of the work: The aim of the bachelor's thesis is to contribute to solving the problem of physiotherapy in a patient after an open fracture of the hand in the area of the wrist and thumb. Methodology: In my bachelor's thesis, I am working with a patient who suffered an open fracture of the hand with an injury to the thumb adductor in an accident. After the initial kinesiology analysis, I developed a short-term physiotherapy plan, in the course of which I used various concepts and methods. Among the most frequently used techniques, I would mention soft tissue techniques, stretching shortened structures, strengthening weakened structures, mobilisation of the wrist and small joints of the hand, increasing the range of motion, increasing muscle strength, grip training, balling, hedgehogs, rhytmical stabilization (RS), proprioceptive neuromuscular facilitation (PNF), postisometric relaxation (PIR) of the forearm and hand muscles. Results: Thanks to regular rehabilitation, there was a significant improvement in muscle strength, range of motion in the given joints and fine motor skills. The scar and the skin, which was covered by a splint for part of the rehabilitation, healed with minor complications (skin irritation, rash). Conclusion: We were able to restore full ranges of motion in the wrist and index finger. The range of motion in the carpometacarpal and metacarpophalangeal joints of the thumb was increased, but not to the full extent. In hand grips, the current range of motion of the thumb is fully sufficient and satisfactory. Key words: adductor thumb, pain, fine motor skills, open fracture of the hand, grip.
