Validation study of Benton's Judgment of Line Orientation on the population of Czech seniors and individuals with Mild Cognitive Impairment

Abstract:

Bentonův test orientace čar (JLO) je běžně užívaným nástrojem při neuropsychologickém vyšetření zrakově-prostorových schopností. Tato studie si klade za cíl ověřit psychometrické vlastnosti testu na populaci českých seniorů a osob s mírnou kognitivní poruchou. Studie vychází ze skórů 192 osob (průměrný věk 68,5 let), z nichž 50 představuje klinickou skupinu osob s mírnou kognitivní poruchou (MCI) a 142 osob bylo zařazeno do skupiny zdravých kontrol (KS). Mezi skupinami byly nalezeny signifikantní rozdíly ve výkonu JLO (p = 0,01). Rozlišovací schopnost testu byla zjišťována pomocí ROC (Receiver-Operating Characteristic) křivky, která ukázala statisticky významnou hodnotu rozlišovací schopnosti JLO (AUC = 0,621; p = 0,011). Test-retest reliabilita byla vypočítána pomocí retestové korelace u části souboru (n = 52) a indikuje středně vysokou test-retestovou reliabilitu (r = 0,607; p < 0,001). Statisticky významné výsledky svědčí o validitě této metody vzhledem ke dvěma dalším testům se zrakově-prostorovou složkou, a sice Testu hodin (r = 0,381; p < 0,01) a kopií Taylorovy komplexní figury (r = 0,316; p < 0,01). Rovněž byly vytvořeny orientační percentilové normy pro muže a ženy u kontrolní skupiny. Závěrem lze shrnout, že JLO je schopen rozlišovat mezi klinickou skupinou a skupinou zdravých osob. Jeho rozlišovací schopnost (AUC = 0,621) představuje přijatelnou hodnotu, vzhledem ke skutečnosti, že poruchy zrakově-prostorových schopností nejsou specifické pro deficit u MCI.

Abstract:

Benton’s Judgment of Line Orientation (JLO) is a commonly used non-verbal diagnostic method for neuropsychological assessment of visuo-spatial skills. This study aims at investigating the psychometric properties of the test for the Czech elderly population and individuals with mild cognitive impairment. The study examines test scores of 192 participants (mean age 68.5 years), 50 of whom had been diagnosed with mild cognitive impairment (MCI) and 142 belonged to the healthy controls group (HC). Significant differences in JLO scores were observed between the two groups (p < 0.05). The discriminatory power of JLO was assessed by ROC (Receiver-Operating Characteristic) curve and showed significant result (AUC = 0.621; p < 0.05). Test-retest reliability was measured by test-retest correlation on a part of the control sample (N = 52) and indicates moderately high test-retest reliability (r = 0.607; p < 0.01). Significant convergent validity was observed with other visuospatial measures: Clock Drawing Test (r = 0.381; p < 0.01) and the copy of Taylor Complex Figure (r = 0.316; p < 0.01). In the control group, approximate percentile norms based for sex have been created. In summary, this study proves that JLO does differentiate between MCI and HC group. The overall discriminatory power (AUC = 0.621) represents an acceptable value, given the fact that visuospatial deficits are not specific for mild cognitive impairment.
