Abstract:

Předmětem disertační práce je pojednání o dysplastických spondylolistézách s vysokým rizikem progrese skluzu. Dysplastické spondylolistézy tvoří podskupinu ze širší skupiny spondylolistéz. Zatímco u ostatních zástupců spondylolistéz je terapeutický postup relativně jednoznačný, u dysplastických je správný postup léčby nejednotný, a často až protichůdný. Práce je rozdělena na teoretickou a klinickou část, které pojednávají o jednotlivých operačních technikách, zhodnocení měřených veličin a sledování subjektivního vnímání operační léčby pomocí dotazníků spokojenosti. Experimentální část se pak zabývá 3D počítačovými modely páteře a zkoumáním vlivu skluzu na biomechanické zatížení postiženého segmentu s posouzením změny zatížení po provedeném operačního výkonu. Cílem práce je retrospektivní zhodnocení souboru pacientů operovaných na Ortopedické klinice FN Brno, sjednocení jednotlivých operačních technik a stanovení ideálního diagnosticko-terapeutického algoritmu. Pomocí experimentální studie se snažíme pak prokázat patologický vliv progredujícího skluzu a prospěšnost operační terapie.

Abstract:

The subject of the dissertation is a discussion about dysplastic spondylolisthesis with a high risk of slippage progression. Dysplastic spondylolisthesis forms a subgroup of the wider group of spondylolisthesis. While for other representatives of spondylolisthesis, the therapeutic procedure is relatively unambiguous, for dysplastic ones the correct treatment procedure is inconsistent and often even contradictory. The work is divided into a theoretical and clinical part, which discusses individual surgical techniques, evaluation of measured parameters and evaluation of the subjective perception of surgical treatment using satisfaction questionnaires. The experimental part deals with 3D computer models of the spine and investiagate the effect of slippage on the biomechanical load of the affected segment with an assessment of the change in the load after the surgery. The aim of the thesis is a retrospective evaluation of the group of patients operated on at the Orthopedic Department, University Hospital Brno and the unification of individual surgical techniques and the determination of an ideal diagnostic and therapeutic algorithm. By means of an experimental study, the pathological influence of the progressive slip and the usefulness of the operative therapy was demonstrated.
