Characterization of mechanisms leading to a synergistic therapeutic effect of phages and antibiotics against staphylococcal infections

Abstract:

Vzhledem k rostoucí hrozbě post-antibiotické éry je třeba hledat alternativní způsoby léčby bakteriálních infekcí. Jednou z možností je kombinace antibiotické a fágové terapie, která v ideálním případě vede k synergickému efektu (Phage Antibiotic Synergy, PAS). Ten byl pozorován u řady bakteriálních druhů. Jedním z nich je Staphylococcus aureus, jehož často multirezistentní kmeny bývají původci závažných infekcí. Aby bylo možné aplikovat PAS v klinické praxi, je nutné charakterizovat mechanismy, jež za tímto efektem stojí. Ačkoliv byl původ synergie u některých kombinací antibiotik a fágů již objasněn, pro řadu jiných kombinací není fungování PAS dostatečně charakterizováno. Nejasný je zejména mechanismus synergie mezi fágy a antibiotiky inhibujícími proteosyntézu. Jelikož jsou fágy závislé na proteosyntéze hostitele, měla by mít její inhibice na životní cyklus fága negativní dopad. Nicméně řada studií popisuje i v tomto případě synergický efekt. Tato práce testuje hypotézu, že PAS může spočívat v inhibici bakteriálních proteinů zapojených do obranných systémů proti fágům, a tím se snaží přispět k objasnění mechanismů interakce fágů a antibiotik inhibujících proteosyntézu. Zároveň se věnuje analýze stability fágů v lékových formách vhodných pro přípravu terapeutik založených na PAS efektu.

Abstract:

Due to the growing threat of the post-antibiotic era, alternative treatments for bacterial infections must be investigated. One option is a combination of antibiotic and phage therapy, which ideally leads to a synergistic effect (Phage Antibiotic Synergy, PAS). This has been observed in a number of bacterial species. One of them is Staphylococcus aureus, whose often multidrug-resistant strains are the causative agents of serious infections. In order to apply PAS in clinical practice, it is necessary to characterize the mechanisms behind this effect. Although the origin of synergy has been elucidated for some combinations of antibiotics and phages, for many other combinations the functioning of PAS has not been adequately characterized. In particular, the mechanism of synergy between phages and antibiotics inhibiting protein synthesis is unclear. Since phages are dependent on the host translational apparatus, its inhibition should have a negative impact on the phage life cycle. However, a number of studies describe a synergistic effect in this case as well. This work tests the hypothesis that PAS may be based on the inhibition of bacterial proteins involved in anti-phage defence systems, and thus attempts to contribute to the elucidation of the mechanisms of interaction between phages and antibiotics inhibiting proteosynthesis. In addition, it is focused on the analysis of phage stability in drug forms suitable for the preparation of therapeutics based on the PAS effect.
