Colistin for the treatment of Gram-negative infections

Abstract:

Bakalářská práce pojednává o toxickém antibiotiku s názvem polymyxin E neboli kolistin. Z důvodu řady nežádoucích účinků se jedná o lék poslední volby používaný u pacientů ve vážném zdravotním stavu, u kterých již jiná léčba nezabírá. Největší problematikou antibiotik je stále narůstající bakteriální rezistence k těmto léčivům. Kromě rezistence se v této práci hovoří mj. o mechanismu působení a nežádoucích účincích kolistinu, dále u kterých bakteriálních infekcí se používá a jak je možné stanovit citlivost bakterií ke kolistinu. Cílem práce bylo optimalizovat metodu stanovení minimální inhibiční koncentrace kolistinu pomocí mikrometody a její ověření u kmenů s pravděpodobnou rezistencí ke kolistinu a podat aktuální přehled výskytu gramnegativních tyčinek s rezistencí ke kolistinu v klinickém materiálu. Optimalizace byla provedena pomocí kontrolních kmenů Escherichia coli CCM 8874 a Escherichia coli CCM 3954. Bylo provedeno 5 variant přípravy inokula, z nichž s ohledem na cílové hodnoty nejlépe vycházela varianta přípravy přímého inokula o hustotě 1 McFarland aplikovaného do mikrotitrační destičky inokulátorem o tloušťce jehly 1 mm. Tato varianta byla ověřena na 10 vybraných kmenech s předpokládanou rezistencí ke kolistinu. Výskyt rezistence ke kolistinu byl sledován u 5 bakteriálních kmenů izolovaných z klinického materiálu (ze sput a moče) v letech 2018-2020. Nejvýraznější nárůst rezistence ke kolistinu byl v obou klinických materiálech pozorován u kmenů Enterobacter cloacae, naopak u kmenů Escherichia coli a Klebsiella pneumoniae byl výskyt za sledované období relativně stálý. Na vzrůstající rezistenci ke kolistinu poukazují data u kmenů Acinetobacter baumannii, izolovaných ze vzorků sput a Pseudomonas aeruginosa, izolovaných ze vzorků moče: rezistence ke kolistinu u obou skupin kmenů byla v letech 2018 a 2019 nulová, záchyt byl až v roce 2020, což poukazuje na využívání tohoto antibiotika v praxi. Rezistence ke kolistinu bude pravděpodobně narůstat i nadále, proto je nutné se touto problematikou stále zaobírat.

Abstract:

This bachelor thesis is focused on a toxic antibiotic called polymyxin E or colistin. Due to the various adverse effects, it is used as a last option for patients who had already exhausted other ways of treatment. The biggest issue concerning antibiotics is the growing bacterial resistance. In addition to this, this thesis is also concerned with colistin's mechanism of action and adverse effects, its use in bacterial infections and the bacterial sensitivity measurement. The aim of this thesis was to optimise a method for establishing the minimal inhibiting concentration of colistin using micromethod and its validation in bacterial strains with predicted colistin resistance. The current summary of the occurrence of Gram-negative rods resistant to colistin in clinical materials is also included. The optimisation was performed using control strains Escherichia coli CCM 8874 and Escherichia coli CCM 3954. Five variants of inoculum preparation were compared and, based on the final values, the best was the preparation of direct inoculum of the density of 1 McFarland applied to microtiter plate using an inoculator with a needle of 1 mm diameter. This method was verified using 10 chosen strains with predicted colistin resistance. The occurrence of colistin resistance was assessed by 5 bacterial species isolated from clinical materials (sputum and urine) between 2018 and 2020. The greatest resistance growth in both clinical materials was shown in the strain Enterobacter cloacae. In contrast, the occurrence in strains Escherichia coli and Klebsiella pneumoniae was stable in the assessed period. The growing colistin resistance is supported by data from strains Acinetobacter baumannii, isolated from sputum samples and Pseudomonas aeruginosa, isolated from urine samples: the resistance in both strains was zero in years 2018 and 2019, however, it appeared in 2020 which correlates with the use of this antibiotic in a practical setting. Colistin resistance will probably continue to grow, which is why this issue needs to be addressed.
