Abstract:

?Lombalgia: confronto tra le tecniche di neurodinamica e le mobilizzazioni lombari? 12 ABSTRACT Low back pain : comparison between neurodynamic techniques and lumbar mobilization Key words: Low back pain (LBP) , lumbar mobilization, neurodynamic test, SLUMP TEST, SLR The following study is still experimentation but addressed to understand and treat a fairly common pathology: the low back pain. The study was accomplished by the comparison between two kinds of treatment techniques: in particular, group A is composed by patients treated with both vertebral mobilization and a program of neurodynamic exercises; group B is composed by patients treated just with vertebral mobilization. Eventually the aim is to demonstrate that when the patients follow the neurodynamic exercises individually at home and without any help by a therapist, they manage to get better and their sintomatology improves; unlike the ones who are just treated with lumbar mobilization. 20 patients were chosen out of a group of 31: their age range is from 31 to 55 years. There were 14 females and 6 males and they presented the right features to enter the study, following the inclusion-exclusion criteria. Each patient was treated with vertebral mobilization twice a week for 5 weeks; group A had to add neurodynamic exercises: SLUMP TEST and SLR. The patients status was examined at the beginning and at the end of each treatment for a short-term valuation and at the beginning of the experimentation and one month later for a long-term valuation. The following valuation sheets were taken into account to estimate the reaction to the two different exercise protocols: - Ortopedic evaluation sheet L.U.De.S - Positivity to the Neurodynamic test - VAS sheet The arm fisioterapic goniometer and the centimeter are tools used to test the ROM improving, after the treatments. Eventually, here are the conclusions and results of this study: - Both the treatments influenced positively the patients sintomatology according to the ?Lombalgia: confronto tra le tecniche di neurodinamica e le mobilizzazioni lombari? 13 indicative VAS value. There are no significant statistic differences between the two groups. - Datas collected from measuring the respective flexibility show that both the groups improved but a statistic better result was reached by group A. - Datas collected from measuring right and left lateral flexion show improvements in both groups without relevant differences, both at the beginning and at the end of the single treatment and at the beginning and at the end of the five weeks of treatments. Considered that both groups had the same number of patients, same amount of exercises and treatments, we can conclude by saying that both groups improved their own VAS and group A managed to achieve a higher level of flexibility thanks to the neurodinamic exercises. This study includes the following limits: a limited number of patients, a short period of treatment and measuring of the range of motion (ROM) with manual tools. This are the reasons why this study is to be considered as a pioneer and preliminary study and needs to be followed by other wider and in-depth studies.

Parole chiave: lombalgia, mobilizzazioni lombari, test di neurodinamica, SLUMP TEST, SLR Lo studio sperimentale č indirizzato alla comprensione e al trattamento di una patologia abbastanza diffusa: la lombalgia. Il lavoro si č sviluppato mettendo a confronto due tipologie di tecniche di trattamento: la prima, nella quale rientra il gruppo A, comprende i pazienti che hanno ricevuto il trattamento di mobilizzazioni vertebrali con in aggiunta il programma di esercizi di neurodinamica , la seconda, nella quale confluisce il gruppo B, riunisce tutti quei pazienti trattati con mobilizzazioni lombari. L?obiettivo finale č di dimostrare che con esercizi di neurodinamica, svolti a domicilio e senza l?aiuto di un terapista, possano migliorare la sintomatologia del paziente, a differenza di mobilizzazioni passive. Sono stati contattati 32 pazienti, tra i 31 e 55 anni, ma solo 20 di questi ( suddivisi in 14 di sesso femminile e 6 di sesso maschile) hanno preso parte al lavoro sperimentale presentando le caratteristiche idonee, secondo i criteri di inclusione ed esclusione. Tutti i pazienti sono stati collocati in modo casuale in uno dei due gruppi di lavoro. Entrambi i gruppi ricevevano il trattamento di mobilizzazioni lombari due volte alla settimana per la durata di cinque settimane, soltanto il gruppo A oltre alle mobilizzazioni doveva seguire a domicilio gli esercizi di neurodinamica: lo SLUMP TEST e SLR. I pazienti venivano valutati ad ogni inizio e fine trattamento, per la valutazione a breve termine e a distanza di un mese per una valutazione a lungo termine. Al fine di validare la risposta ai due differenti protocolli di lavoro sono state utilizzate alcune schede di valutazione: - Scheda di valutazione ortopedica L.U.De.S. - Positivitŕ dei Test Neurodinamici - L?Oswestry Low Back Pain Disability Questionnaire - Scheda VAS Per poter valutare un miglioramento del Range of Motion (ROM) del paziente a seguito del trattamento sono stati utilizzati il goniometro fisioterapico a bracci e il centimetro. I risultati seguenti, una volta terminato il lavoro sperimentale, permettono di giungere a queste ?Lombalgia: confronto tra le tecniche di neurodinamica e le mobilizzazioni lombari? 11 conclusioni: - La sintomatologia del paziente č migliorata in entrambi i gruppi di lavoro. Per mezzo dell?indicativo valore VAS, non sono emerse differenze statisticamente significative tra i due gruppi. - I dati raccolti sulle relative misure di flessione indicano un miglioramento effettivo della flessione di entrambi i gruppi, tuttavia un?evidenza statistica migliore č stata riscontrata nel gruppo di lavoro A. - La raccolta delle misure di inclinazione destra e sinistra, indica, invece, un effettivo miglioramento in entrambi i gruppi ma non si evidenziano differenze statisticamente rilevanti tra i valori misurati a breve termine e quelli misurati a lungo termine. In conclusione, a paritŕ di numero di soggetti appartenenti ai due gruppi di studio, oltre che di numero di trattamenti dei due protocolli, si č osservato un miglioramento della sintomatologia del paziente, valutata con la scala di valutazione VAS, in entrambi i gruppi di lavoro e un miglioramento del parametro della flessione nel gruppo A trattato con le tecniche di neurodinamica. Tale studio presenta i seguenti limiti: ridotte dimensioni del campione, ridotto tempo di trattamento e misurazione del Range of Motion (ROM) con strumenti manuali. Per tali ragioni questo studio č da considerarsi uno studio pilota preliminare, e vuole essere punto di partenza per futuri lavori in grado di superare i limiti sopraesposti.
