Evaluation of immune regulatory cells and their clinical significance in patients with multiple myeloma: a detailed phenotypic, functional and molecular characterization study

Abstract:

Mnohočetný myelom (MM) je maligní onemocnění plazmatických buněk (PC), které se hromadí v kostní dřeni (KD), což vede k anémii, poškození kostí, hyperkalcémii, renálnímu poškození a imunitní dysfunkci. T regulační (Treg) buňky udržují imunitní homeostázu u zdravých jedinců tím, že hlídají nežádoucí imunitní reakce. Ale u pacientů s nádorovým onemocněním jsou tyto buňky nekontrolovatelně expandované a tím tlumí hostitelské protinádorové odpovědi. Tato disertační práce je zaměřena na pochopení role Treg buněk, a to jak CD4 Treg tak i CD8 Treg buněk, které ovlivňují imunitní postižení u MM pacientů. Dále, tato práce chtěla odhalit klinický a prognostický význam Treg buněk, jakož i jejich změny po léčbě imunomodulačními (IMiD) léky a dexametazonem. Stručně řečeno, tyto nálezy naznačují, že CD4 Treg a CD8 Treg buňky jsou zvýšené a supresivní u MM pacientů a tím podporují imunitní poškození a mohou zvýšit infekční komplikace u MM pacientů. Je možné, že Treg buňky jsou jeden z únikových mechanismů myelomových buněk z hostitelské imunitní odpovědi, protože jejich nárůst způsobuje postupnou progresi onemocnění u MM pacientů. Kombinace IMiD s kortikosteroidy pravděpodobně snižuje imunologické účinky IMiD přes zvýšení Treg buněk.

Abstract:

Multiple myeloma (MM) is a malignancy of plasma cells (PCs) which accumulate in the bone marrow (BM), thus, leading to anemia, bone lesions, hypercalcemia, renal impairments and immune dysfunction. T regulatory (Treg) cells maintain immune homeostasis in healthy individuals by patrolling unwanted immune responses. Whereas in cancer patients these cells are uncontrollably expanded, thereby, inhibiting the host anti-tumor responses. Majorly this thesis work focused to understand the role of Treg cells, including CD4 Treg cells and CD8 Treg cells in rendering the immune impairments in MM patients. In addition, this work aimed to reveal the clinical and prognostic significances of Treg cells, as well their changes after treatment with immunomodulatory (IMiD) drug plus dexamethasone. In summary, this study findings clearly suggest that CD4 Treg cells and CD8 Treg cells in MM patients are increased and suppressive, thereby, support the immune impairment and may enhance the infectious complications in MM patients. It could be considered that Treg cells as one of the evasion mechanisms of myeloma cells from host immune responses because their increase causes disease progression in MM patients. Combination of IMiD with corticosteroid seems to dilute the immunological effects of IMiD via increasing the Treg cells.
