Physiotherapy at a client with morbus Bechterew

Abstract:

Název: Fyzioterapie u klienta s morbus Bechtěrev Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u klienta s onemocněním morbus Bechtěrev s využitím vhodných fyzioterapeutických postupů, konceptů a metod. Metodika:V Hlavní části jsem zpracovala kazuistiku klienta s diagnózou morbus Bechtěrev 2. - 3.stupně. Na základě vstupního kineziologického rozboru (vyšetření dynamiky páteře, vyšetření dechové exkurze, goniometrie) jsem sestavila krátkodobý fyzioterapeutický plán. Fyzioterapeutická intervence proběhla během 3 měsíců (12 terapií), v rámci fyzioterapeutické intervence jsem využila fyzioterapeutických konceptů a metod ? myofasciální techniky, mobilizační techniky, dechová cvičení, SMsystém, základy Brügger konceptu, prvky sensomotorické stimulace, PNF, ACT?. Po 6 týdnech, jsem provedla kontrolní vyšetření obvodů hrudníku, vyšetření páteře a goniometrii. Na poslední terapii jsem provedla výstupní kineziologický rozbor, na základě toho sestavila dlouhodobý fyzioterapeutický plán a doporučila režimová opatření. Výsledky: Došlo k odstranění všech patologických bariér uvedených fascií a k odstranění téměř všech palpovaných Trigger pointů.Amplituda hrudníku se zvětšila o 6 cm. Zlepšení pohyblivosti kořenových kloubů je znatelné převážně v konečných fázích rozsahů pohybů (dále jen RP), kdy největší rozdíly se týkají RP do flexe pasivně o 10°, zevní rotace o 5°, RP do extenze o 10°. U kyčelního kloubu se zvětšila RP do flexe na PDK o 5°, extenze na obou DKK o 5, abdukce o 5°. V rámci goniometrického vyšetření Cp jsem zjistila toto zlepšení: lateroflexe pasivně vpravo o 5°, aktivně vpravo o 5°. Rotace pasivně i aktivně vlevo o 5°, pasivně i aktivně vpravo o 15°. U zkoušek pohyblivosti páteře došlo k těmto změnám: Stiborův index o 2 cm, Čepojův příznak o 0,5 cm, Ottův inklinační index o 1 cm, Ottův reklinační index o 1,5cm, Thomayerův příznak o 6cm a zkouška lateroflexe o 1cm. Závěr:V rámci zpracovávání této bakalářské práce, jsem se přesvědčila, že pohyb pro pacienta s AS je opravdu velmi důležitý a komplexní péče významně zlepšuje kvalitu života pacientů

Abstract:

Aim: The aim is focus on physiotherapy for a client with Morbus Bechterew using optimal physiotherapeutic procedures, concepts and methods. Methodology:In the special part there is a case report of a client diagnosed with morbus Bechterew 2. -3. st. Based on the input kinesiological analysis (which included examination of spine dynamics, examination of breathing excursion, anthropometry), I compiled a short-term physiotherapy plan.Physiotherapeutic intervention lasted 3 months. In the framework of physiotherapy intervention I used different concepts and methods - myofascial techniques, mobilization techniques, breathing exercises, SMsystem, Brügger concept, sensomotor stimulation, PNF (Proprioceptive Neuromuscular Facilitation), ACT (Acral Coactivation Therapy). In the middle of our intervention, after 6 weeks, I made a control examination focused on the chest circumference, range of motion of spine, anthropometry. At the last therapy I made a output physiotherapeutic analysis and based on that I compiled a long-term physiotherapy plan and recommended regime measures. Results:All pathological barriers of all listed fascias were removed and almost all palpated Trigger points were removed. The excursion of the chest was increased by 6 cm. Improvement of mobility on the upper and lower limbs mainly concerns the final stages of the range of motion - flexion increased passively by 10 °, external rotation by 5 °, extension by 10 °. In the hip joint, the flexion of the right lower limb increased by 5 °, the extension on both hip by 5, and abduction by 5 °. In the goniometric examination of the cervical spine I found the following improvement: lateroflexion passively right 5 °, actively right 5 °. Rotation passively and actively to the left by 5 °, passively and actively to the right by 15 °. The following changes were observed in the spine mobility tests: Stibor index by 2 cm, Cepoj's symptom by 0.5 cm, Otto's inclusion index by 1 cm, Otto's reclining index by 1.5 cm, Thomayer's symptom by 6 cm, and lateroflex test by 1 cm. Conclusion: In the preparation of this thesis, I was convinced that movement for patient with ankylosis spondylitis is really very important and comprehensive care significantly improves the quality of life of patients with ankylosing spondylitis. This work has certainly become a benefit for my further practice.
