Miniinvasive Osteosynthesis and Transplantation of the Autogenous Chondrocyte Graft to the Therapy of the Physeal Defects

Abstract:

Práce je rozdělena do 2 částí, klinické a experimentální. Cílem klinické části práce je detailní vyhodnocení výsledků terapie poranění distálního segmentu femuru u pacientů léčených na Klinice dětské chirurgie, ortopedie a traumatologie (KDCHOT) a současně navržení optimálního algoritmu terapie těchto poranění k minimalizaci vzniku komplikací, zejména vzniku kostního můstku. Cílem experimentální části práce bylo propracování techniky transplantace autogenních chondrocytárních graftů do defektů v oblasti růstové ploténky a propracování techniky kombinace osteosyntézy napříč růstovou ploténkou s transplantací autologních chondrocytárních graftů tak, aby v nutných případech byla klinicky aplikovatelná. Za experimentální zvíře jsme použili prase domácí. Experiment jsme prováděli na distální růstové ploténce femuru. Statistické hodnocení prokázalo, že metoda autogenní chondrocytární transplantace do defektu v oblasti růstové ploténky je vysoce účinná a bylo by možné jí využít a zavést do klinické praxe. Současně se statisticky potvrdila účinnost techniky kombinace osteosyntézy napříč růstovou ploténkou s transplantací autologních chondrocytárních graftů a tato metoda léčby poranění růstových chrupavek je tedy v nutných případech klinicky aplikovatelná.

Abstract:

The postgraduate study is divided into two parts, clinical and experimental. The aim of the clinical study is a detail analysis of results of distal femoral segment s injuries by patients treated at the Clinic of pediatric surgery, orthopedic and traumatology [KDCHOT] and contemporary the suggestion of optimal therapeutic algoritmus of these injuries to the minimalization of a complications rise, especially the bone bridge s rise. The aim of the experiment was to elaborate a technique of autogenous chondrocytes grafts implantation in damaged physis and elaborate a technique of the osteosynthesis across the physis with the autogenous chondrocytes grafts implantation so as to be clinically applied in needed cases. A domestic swine was chosen as an experimental animal. The experiment was performed on the growth plate of the distal femur. The statistical evaluation proved that the method of autogenous chondrocytes implantation in the growth bone plates defect is high effective and it is possible to exploit and establish it into a clinical practice. The influence of the technique of the osteosynthesis through the physis with autogenous chondrocytes graft s implantation was statistically validated and this therapeutic method of growth plates injuries is in necessary cases clinically possible to apply.
