Nursing Care Plan for Patient after Urinary Diversion

Abstract:

Bakalářská práce je zaměřena na oblast urologie. Cílem práce je zpracovat návrh plánu ošetřovatelské peče pro pacienta po močové derivaci. Práce obsahuje dvě části. Teoretická část se zabývá stručně anatomií močového systému, popisuje základní rozdělení močových derivací. Podrobněji se práce zabývá močovými derivacemi jiným orgánem, jejích druhy, indikacemi k těmto derivacím a komplikacemi, které mohou po operaci nastat. Praktická část je zaměřena na plán ošetřovatelské péče v předoperačním, intraoperačním a pooperačním období u pacienta po močové derivaci se založením ureteroileostomie. K vytvoření plánu péče je využit ošetřovatelský model Marjory Gordonové, který je z hlediska holistické filosofie nejkomplexnější. Je také nazýván jako model funkčního typu zdraví, který je odvozen z interakce člověka s prostředím. Zdraví se vyjadřuje jako integrace biologických, psychologických a sociálních potřeb. Pokud dojde k poruše v jedné oblasti, je narušena rovnováha zdraví celého organismu. Model je rozdělen do dvanácti oblastí. Každá oblast obsahuje funkční a dysfunkční součást zdravotního stavu. Pro dysfunkční typ zdraví je typické, že sestra při péči o pacienta stanoví ošetřovatelské diagnózy podle priorit a na jejich základě stanoví plán péče. Při vypracování plánu jsou v práci použity ošetřovatelské diagnózy v NANDA II doménách.

Abstract:

The bachelor thesis is directed to the field of urology. Its aim is to propose a plan of nursing care for patients after urinary diversion. The thesis consists of two parts. The theoretical part deals with the anatomy of urinary system and describes the basic classification of urinary diversions. The thesis addresses in more details the urinary diversions by other organ, its types, indications to these diversions and complications that may set in after operation. The practical part is directed to the plan of nursing care during preoperative, intraoperative and postoperative periods for patient after urinary diversion by ureteroileostomy. The Marjory Gordon Nursing Care Model has been used for the preparation of the care plan. This model is the most complex from the holistic philosophy point of view. It is also called the model of functional type health and is deduced from the human ? environment interaction. Health is expressed as a bio-psycho-social integration. If a disorder occurs in one area, then the equilibrium of the whole organism health is disturbed. The model is divided into twelve areas. Every area includes functional and dysfunctional parts of the state of health. It is typical for the dysfunctional type of health that during the care, a nurse determines nursing diagnoses according to priorities and establishes the plan of care based on these diagnoses. In the thesis, the nursing diagnoses in NANDA II domains are used for the preparation of the plan.
