Intraoperative fluorescence using 5-aminolevulinic acid in patients with high grade glioma. Its influence on the radicality of the glioma resection and its correlation with histological and imaging methods

Abstract:

Cílem této práce bylo zkoumat vztah mezi intenzitou intraoperační fluorescence tkáně a histopatologickým nálezem u pacientů s glioblastomem. Všimli jsme si, že v operačním poli během fluorescencí navigované resekce glioblastomu můžeme rozlišit tři úrovně intenzity fluorescence: 1. červená, silně intenzivní fluorescence; 2. růžová, slabě intenzivní fluorescence; 3. tkáň bez fluorescence. Provedli jsme prospektivní studii u 13 pacientů s glioblastomem, kteří byli operováni s použitím fluorescence. Histologickým vyšetřením reprezentativních vzorků z jednotlivých úrovní intenzity fluorescence bylo zjištěno, že intenzita fluorescence závisí na relativním podílu nádorových buněk ve tkáni. Prokázali jsme statisticky významnou korelaci mezi intenzitou fluorescence a relativním podílem maligních buněk ve tkáni (p < 0.001), a zjistili vysokou prediktivní hodnotu fluorescence na přítomnost maligních buněk (PPV = 92%). Taktéž jsem zaznamenali, že i další histopatologické znaky malignity jsou v okrajové části glioblastomu méně vyjádřeny, což upozorňuje na další výzkum této oblasti často ponechané na místě po resekci glioblastomu.

Abstract:

The aim of this study was to investigate the relationship between the intraoperative fluorescence intensity and the histopathological findings. We noticed that three levels of fluorescence intensity can be distinguished in the operative field during the fluorescence guided resection of glioblastoma using 5-ALA: 1. red, highly intensive fluorescence zone; 2. pink, moderately intensive fluorescence zone; 3. none, tissue without the fluorescence. We have conducted a prospective study of 13 patients with glioblastoma who were operated on using the fluorescence guidance. The histopathological examination of the representative specimens of the different fluorescence intensity levels revealed that the fluorescence intensity depends on the relative proportion of malignant cells in the tissue. We have found statistically significant correlation between the fluorescence intensity and the relative proportion of malignant cells (p value < 0.001), and calculated a high positive predictive value of the fluorescence for the presence of the malignant cells (PPV = 92%). We have also observed that the presence of other histopathological changes typical for glioblastoma are gradually less pronounced at the tumor margin, and the further research of this area commonly left behind after the glioblastoma resection is recommended.
