Comprehensive rehabilitation after planned re-surgery of anterior cruciate ligament of the knee joint in the handball player

Abstract:

Cíl: Cílem této práce je přispět k řešení problematiky volby vhodných fyzioterapeutických postupů, metod a konceptů u pacienta s plánovanou re-plastikou předního zkříženého vazu. Metodika: Na zpracování kazuistiky pacienta po re-plastice LCA jsem získala data pomocí metod rozhovoru, pozorování a měření. Byla odebrána anamnéza a proveden vstupní kineziologický rozbor. Vyšetření bylo zaměřeno na antropometrické a goniometrické měření, hodnocení svalového tonu a svalové síly, vyšetření zkrácených struktur a speciálních testů, dále vyšetření hlubokého stabilizačního systému a chůze. Na základě těchto údajů byl zpracován krátkodobý fyzioterapeutický plán a následně zvoleny vhodné postupy fyzioterapie- techniky měkkých tkání, postfacilitační strečink, rytmická stabilizace, ošetření zkrácených struktur, mobilizace kloubu, aktivní cvičení s overballem, gymballem, therabandem, využití akrální koaktivační terapie (ACT), proprioceptivní neuromuskulární facilitace (PNF), senzomotorické stimulace a metody R. Brunkow. Byly provedeny dvě kontrolní měření - po 3 a 5 týdnech. Po skončení fyzioterapeutické intervence byl vypracován výstupní kineziologický rozbor zaměřený na antropometrické a goniometrické měření, hodnocení svalového tonu a svalové síly, vyšetření zkrácených struktur a speciálních testů, dále vyšetření hlubokého stabilizačního systému a chůze. Dále byl vypracován dlouhodobý fyzioterapeutický plán. Výsledky: Na základě zmíněných metod v kombinaci domácího cvičení a ambulantních rehabilitací došlo k uvolnění kloubních a svalových struktur, zhojení jizev per primam, redukci otoku, zvětšení ROM, obnovení správného stereotypu chůze v domácím i externím prostředím. Navrátila se stabilita kloubu a fyzioterapie měla pozitivní účinky. Závěr: Při správném výběru operačního přístupu a následné včasné a cílené fyzioterapii dochází k úspěšnosti léčby a navrácení pacienta zpět do běžného života, rekreačním sportům, ale i ke sportovní kariéře. Celá tato intervence byla pro pacienta velmi přínosná.

Abstract:

Aim: The aim of this bachelor thesis is to contribute to the solution of the choice of suitable physiotherapeutic procedures, methods and concepts in a patient with planned re-surgery of the anterior cruciate ligament. Methodology: For the treatment of the patient's case after LCA re-surgery, I obtained data using the method of interview, observation and measurement. Anamnesis was taken and initial kinesiological analysis was proved. The examination was focused on anthropometric and goniometric measurements, evaluation of muscle tone and muscle strength, examination of shortened structures and special tests, further examination of the deep stabilization system and gait. Based on these data, a physiotherapeutic plan was elaborated and physical procedures of physiotherapy were performed - soft tissue techniques, post-facial stretching, rhythmic stabilization, treatment of shortened structures, joint mobilization, active exercise with overball, gymball, theraband. used to acral coaktivation therapy (ACT), the proprioceptive neuromuscular facilitation (PNF), the senzomotor stimulation and the copncept of the Brunkow method. Two control measurements were demonstrated - after 3 and 5 weeks. After physiotherapeutic interventions, a targeted kinesiological analysis focused on anthropometric and goniometric measurements, evaluation of muscle tone and muscle strength, examination of shortened structures and special tests, further examination of the deep stabilization system and gait. Another was elaborated long-term physiotherapeutic plan. Results: Based on these methods in combination with autotherapy at home and outpatient rehabilitation has release of joint and muscles structures, the healing of scars per primam, the reduction of edema, the achieved the enlargement of ROM and the recovery of the right stereotype of walking internal and external environments. The stabilization of joint has returned and physiotherapy had positive effects. Concusion: With the right choice of surgical approach and subsequent timely and targeted physiotherapy, the treatment is successful and the patient returns to normal life, recreational sports, but also to a sports career. The whole intervention was very beneficial for the patient.
