Possibilities of control and prevention of new multi-resistant bacteria in the health facility

Abstract:

Rezistence bakterií k antibiotikům je celosvětově rozšířený problém týkající se nejen zdravotnických zařízení. Objevem antibiotik byla nalezena možnost léčby dříve nevyléčitelných onemocnění, dnes je ovšem problém v jejich nadměrném používání, které způsobuje selekční tlak. Závažným problémem je výskyt rezistentních bakterií ve zdravotnických zařízeních, kde často pobývají imunosuprimovaní pacienti, u kterých by mohla kolonizace těmito bakteriemi způsobit závažné až fatální následky, proto je nutné zavést účinná opatření k zabránění jejich dalšího šíření. V teoretické části této práce se zabývám rezistencí, kde popisuji selekční tlak, genetický podklad a mechanismy vzniku rezistence a především její prevenci. Dále se věnuji problematice CPE, tedy enterobakteriemi produkujícími karbapenemázy, které způsobují odolnost těchto bakterií vůči karbapenemovým antibiotikům, a popisuji možnosti jejich kontroly a prevence. Pro bližší uvedení do problematiky CPE se v práci také stručně zabývám čeledí Enterobacteriaceae a hlavními zástupci, kteří si tento způsob rezistence osvojili. V praktické části byla dotazníkovým šetřením zkoumána informovanost problematiky rezistence a CPE u nelékařských zdravotnických pracovníků ve Fakultní nemocnici v Plzni.

Abstract:

Bacterial resistance to antibiotics is a widespread problem not only for healthcare facilities. The discovery of antibiotics has found the possibility of treating previously incurable diseases, but today there is a problem in their excessive use, which causes selection pressure. A serious problem is the occurrence of resistant bacteria in health care facilities, where immunosuppressed patients often stay themselves in whom colonization with these bacteria can cause serious or fatal consequences, therefore it is necessary to put in place effective measures to prevent their further spread. In the theoretical part of this work I deal with resistance, where I describe selection pressure, genetic background and mechanisms of resistance development and especially its prevention. Furthermore, I deal with the issue of CPE, i.e. carbobenzenase-producing enterobacteria, which cause the resistance of these bacteria to carbapenem antibiotics, and describe the possibilities of their control and prevention. For a more detailed introduction to CPE, I also briefly deal with Enterobacteriaceae and the main representatives of this type of resistance. In the practical part, the questionnaire survey investigated the issue of resistance and CPE in non-medical healthcare workers at the Faculty Hospital in Pilsen.
