Fluid Balance in Intensive Care

Abstract:

Diplomová práce se zabývá zhodnocením bilance tekutin u pacientů na jednotce intenzivní péče. Práce je koncipována jako kvalitativní analýza dat. Práce je zaměřena na porovnání naměřené bilance tekutin počítané na Klinice anesteziologie, resuscitace a intenzivní medicíny Fakultní nemocnice v Ostravě s fyziologickou bilancí tekutin. Fyziologická bilance tekutin zahrnuje tzv. nepočitatelné příjmy (oxidace živin) a nepočitatelné ztráty, mezi něž patří perspirace, respirace a ztráty vody v podobě stolice. Sekundárním cílem práce je vysledovat, zda přítomnost otoků a ran ovlivňuje bilanci tekutin u kriticky nemocných pacientů. Problematika bilance tekutin byla teoreticky zpracována na základě poznatků z literatury, odborných časopisů a internetových zdrojů a publikací. Podstatou zjištění fyziologické bilance bylo zaznamenat tyto parametry: ventilaci pacienta (spontánní nebo umělá plicní ventilace), kolísání tělesné teploty v průběhu dne (hodinový záznam, teplota se liší od každého stupně), přítomnost stolice (formovaná nebo průjmovitá), přítomnost rány (uzavřené, otevřené) a otoků (masivní, na periferii). Do výzkumu bylo zahrnuto 15 pacientů sledovaných po dobu deseti dní. Pacienti byli rozděleni do tří skupin podle diagnóz (polytrauma, respirační selhání, kardiální selhání). Byl porovnáván rozdíl mezi naměřenou kumulativní bilancí a fyziologickou kumulativní bilancí tekutin, dále vztah mezi bilancí tekutin a diagnózou, délkou hospitalizace, přítomností otoků a rány. Ve výzkumu byl zjištěn rozdíl mezi naměřenou a fyziologickou kumulativní bilancí tekutin, dále bylo zjištěno, že není vztah mezi bilancí tekutin a druhem diagnózy, ani délkou hospitalizace. Naopak přítomnost otoků je přímo závislá na bilanci tekutin, ale přítomnost rány bilanci neovlivňuje.

Abstract:

The diploma thesis deals with the fluid balance evaluation of patients on intensive care unit (ICU). The thesis is based on quality data analysis and focuses on comparation of measured fluid balance and physiological fluid balance. The research took a place in Anaesthesiology and resuscitation clinic in University Hospital Ostrava. Physiological fluid balance includes non-measurable intakes (nutrient oxidation) and loses (perspiration, respiration and water loss in form of stool). Another goal is to observe, whether swelling or wound presence influence the fluid balance by critically ill patients. The issue of fluid balance was theoretically based on professional literature, journals and web sources. The basic principle of the physiological fluid balance was to record following parameters: patient ventilation (spontaneous or artificial pulmonary ventilation), fluctuation of body temperature during the day, stool presence and wound and swelling presence. The research included 15 patients observed during the period of ten days. The patients were divided into three groups based on the diagnosis (polytrauma, respiratory failure and cardiac failure). The comparation included the difference between measured and physiological cumulative fluid balance and the relationship between fluid balance, diagnosis, length of hospitalization and swelling or wound presence. The difference was found between measured and physiological cumulative fluid balance. Another finding was, that there is non-existent relationship between fluid balance, diagnosis nor length of hospitalization. Conversely, there is direct dependency between fluid balance and swelling presence, however the wound presence does not affect fluid balance at all.
