Achilles tendon rupture - healing and return to movement

Abstract:

Tato bakalářská práce se týká tématu ruptury Achillovy šlachy a jejího následného léčení. V teoretické části se seznámíme s anatomií, kineziologií, etiologií ruptury a její léčbou. Praktická část zahrnuje dvě kazuistiky, kde porovnávám vstupní vyšetření s výstupním a sleduji průběh terapie. Největším omezením u jednoho z pacientů je otok a snížený rozsah pohybu. U druhého pacienta bylo zásadním problémem nesprávné odvíjení chodidla. Výstupní vyšetření udávají zlepšení stavu obou pacientů ve všech složkách, ale pouze u jednoho z nich se vrátila svalová síla v plné míře. Další data jsou nasbírána pomocí dotazníkového šetření, z něhož vyplývá, že nejčastější příčinou poranění je sportovní aktivita. Ve většině případů pacienti udávají slyšitelné prasknutí doprovázené bolestí. Mezi nejvyužívanější rehabilitační cviky patří krčení prstů, aktivní pohyby hlezenního kloubu, výpony na špičky a v pozdní fázi i rotoped. Pacienti používali také balanční pomůcku Bosu a odporovou gumu Theraband. V oblasti fyzikální terapie se nejvyužívanější procedurou jeví vodoléčba. Časový horizont návratu ke sportu se pohybuje kolem šesti měsíců a pouze v 38% případech ruptura Achillovy šlachy nezanechala trvalé následky.

Abstract:

This bachelor thesis concerns the topic Achilles tendon rupture and its treatment. In the theoretical part we get acquainted with the anatomy, kinesiology, etiology of rupture and its treatment. The practical part includes two case reports, where I compare the input examination with the output and watch the course of therapy. The biggest restriction in one of the patients is swelling and a reduced range of motion. In the second patient, an incorrect unwinding of the foot was a fundamental problem. Output examinations indicate improvement of both patients in all components, but only one of them returned muscle strength to fully. Additional data is collected by a questionnaire survey, from which the most common cause of injury is sports activity. In most cases, patients give an audible burst accompanied by pain. The most used rehabilitation exercises include fingercrouching, active holling joint movements, peak litters and in late phase bicycle. Patients also used Bosu and resistance Theraband. In the field of physical therapy, hydrotherapy seems to be the most used procedure. The time horizon returning to sport is around six months and only in 38% cases Achilles tendon rupture did not leave permanent consequences.
