Nutrition in bariatric surgery patients

Abstract:

Moderní doba se neustále potýká s přetrvávajícím nárůstem zejména dětské obezity a morbidní obezity u dospělé populace. Multifaktoriální příčiny jsou předmětem mnoha výzkumů specialistů různých odborností. Všeobecně se má za to, že jsou to genetické predispozice v kombinaci s „obezitogenním prostředím“. Bylo vyvinuto již mnoho způsobů a postupů jak konzervativní, kognitivně behaviorální, tak farmakologické terapie obezity, které fungují velmi individuálně u každého člověka a v dlouhodobém horizontu velmi často selhávají. Ke komplexní terapii obezity se v posledních desetiletích přidala i bariatrická miniinvazivní chirurgie, s dobrým a dlouhodobým efektem. V teoretické části naši práce ve zkratce popíšeme historii bariatrie, typy, indikace a kontraindikace jednotlivých výkonů. Ve stěžejní části formulujeme doporučení nutričních postupů před a po bariatrickém výkonu. V praktické části na kazuistice pacientky ilustrujeme cestu od „morbidní obezity k lehčímu stupni obezity“ za pomoci operace.

Abstract:

Modern times are still dealing with persisting increase of mainly children obesity and morbid obesity in adult population. Their multifactorial causes are subject of many studies, carried out by experts in various specialisations. In general, these are genetic predispositions, combined with „obesity environment“. Many ways, methods and procedures have been developed, using both conservative, cognitive behavioral treatment of obesity, or/and pharmacological therapy, which both have a very individual effect on every single person, and in a long term perspective they often tend to fail. In the past decades the complex treatment of obesity has been joined by bariatric mini-invasive surgery, which has revealed very good long-term effect. The theoretical part of this study will include a short description of history of bariatric surgery, its various kinds, indications and contraindications for each particular operation. The main part will define nutrition/ diet before and after bariatric surgery, discuss various data on possible malalabsorbtion and nutritional deficiencies in long term follow up. The second part, illustrated by an example of a certain patient, will describe a way from morbid obesity to a lower degree of obesity, achieved by bariatric surgery.
