Health care costs in terms of its centralization in the Czech Republic in a specific diagnosis

Abstract:

Diplomová práce rámcově popisuje současný systém poskytování zdravotní péče v ČR a způsob jejího financování a v návaznosti na to prověřuje dostupnost dat o nákladech, se zaměřením především na "centrovou" péči, tzn. poskytovanou ve specializovaných centrech a analyzuje vývoj těchto nákladů. Práce uvádí příčiny nákladové variability a růstu nákladů ve zdravotnictví. Práce zkoumá stav centralizace zdravotní péče v České republice, členění a počty center, jejich ustavování a mechanismus úhrady za péči v nich poskytované. Dále se, na příkladu léčby karcinomu pankreatu (slinivky břišní), zabývá vztahem mezi kvalitou, náklady a výsledky zdravotní péče z pohledu centralizace. S republikovými daty Národního referenčního centra a za použití metody, založené na zahraničních zkušenostech, ověřuje vztah mezi objemem výkonů a výsledky péče, analyzuje vypočtené hodnoty. Na základě zjištění uvádí doporučení pro české zdravotnictví.

Abstract:

The dissertation describes current system of providing health care in the Czech Republic and a method of its financing followed by cost data availability. It focuses on so-called centre-care which means care provided in specialized centers. It analyzes development of these costs, it states the reasons of costs variables and expenses increase in health care. The paper investigates the state of health care centralization in the Czech Republic, divisions and a number of centers, their establishing and methods of payment for the care provided. Further, like for example pancreatic cancer treatment, it deals with relationship among quality, costs and results of health care from the centralization point of view. With the nationwide data of National Reference Centre and with an application of methods based on foreign countries experience it validates the relationship between a volume of interventions and healthcare outcomes, analyses calculated results. Based on finding it presents recommendations for Czech healthcare.
