Current methods of evaluating the results of treatment of chronic myeloid leukemia

Abstract:

V této diplomové práci se věnuji tématu Současné metody hodnotící výsledky léčby chronické myeloidní leukémie, které navazuje na mou bakalářskou práci Stochastické modely predikující výsledky léčby chronické myeloidní leukémie. Chronická myeloidní leukémie je způsobena translokací chromozomů 9 a 22, kdy vzniká fúzní onkogen BCR - ABL, který způsobuje pak nekontrolovatelné množení leukemických buněk, kdy není třeba jiné aktivace tohoto genu, ať už zvenku nebo zevnitř. Objevem v této oblasti jsou inhibitory tyrosinkináz, které potlačují funkci právě genu BCR - ABL. Existují inhibitory tyrosinkináz první a druhé generace. Z té první je to imatinib, ze druhé pak nilotinib a dasatinib. Nově je nyní stále ve studii bosutinib. Standardně se nasazuje nejdříve imatinib, jehož léčba je pak hodnocena kritérii, které stanovuje společnost European Leukemia Net (ELN). Mým cílem bylo vyhodnotit vliv rizikových faktorů na nový parametr přežití ATFS (alternative treatment - free survival - přežití bez přechodu na jinou léčbu) a ověřit význam ELN kritérií při léčbě imatinibem. Významnost rizikových faktorů jsem hodnotila pomocí Kaplan - Meierovy metody, dále pak jejich vliv na ATFS pomocí Coxova regresního modelu proporcionálních rizik. Dále jsem provedla landmark analýzu, která měla ověřit významnost ELN kritérií v rámci parametru ATFS, čili v přechodu na jinou alternativní léčbu. Tuto analýzu jsem taktéž prováděla pomocí Kaplan - Meierovy metody. Pracovala jsem s daty pacientů z České republiky sledovaných v projektu CAMELIA.

Abstract:

This diploma thesis deals with topic of Current methods of evaluating the results of treatment of chronic myeloid leukemia that develops my bachelor thesis Stochastic models for prognoses of therapeutic results in patiens with chronic myeloid leukemia. Chronic myeloid leukemia is caused by translocation of 9th and 22nd chromosome, where the BCR - ABL fusion oncogene is formed. This fusion oncogene causes uncontrollable multiplying of leucemic cells so there is neither internal nor external impulse needed to activate this gene. In this field is popular a discovery of tyrosine kinase inhibitors, which suppress the function of the BCR – ABL gene. There are tyrosine kinase inhibitors of the first and the second generation. The inhibitor from the first generation is the Imatinib and the inhibitors from the second generation are called Dasatinib and Nilotinib. Newly, the Bosutinib is being studied. Patients are first treated with imatinib. Its treatment is evaluated by kriteria of European Leukemia Net (ELN). I was supposed to evaluate the influence of risk factors on the new parameter ATFS (alternative treatment - free survival) and verify the importance of ELN criteria in its influence on the treatment with imatinib. The importance of risk factors was calculated by Kaplan - Meier method, then its influence to ATFS with Cox regression model of proportional hazards. The Landmark analysis should verify importance of ELN criteria to ATFS, to change to alternative treatment. It was also calculated with Kaplan - Meier method. I worked with data of patients from Czech Republic from project CAMELIA.
