The use of neuropsychological tests for early detection of mild cognitive impairment

Abstract:

Úvod: Mírná kognitivní porucha je přechodný stav mezi změnami kognitivních funkcí ve stáří a demencí. Je to stav, při kterém ale nejsou narušeny běžné denní aktivity jedince. Lidé s touto poruchou většinou trpí zapomnětlivostí, mají potíže naučit se a zpracovat nové informace, objevují se u nich také problémy s koncentrací. Avšak všechny tyto příznaky nedosahují úrovně demence a považují se spíše za mírné. Mírnou kognitivní poruchu nelze jednoznačně diagnostikovat na základě pár symptomů. Změny kognitivních funkcí lze zjistit pomocí neuropsychologických testů. Jedním z nejpoužívanějších testů je Montrealský kognitivní test. Cíl: Cílem této práce bylo analyzovat jednotlivé úlohy v Montrealském kognitivním testu. Zjistit, ve kterých úlohách častěji chybovaly případy a kontroly a porovnat úspěšnost případů a kontrol v řešení úloh. Metodika: Data byla získána z dotazníkového šetření v rámci proběhlé studie. Jednalo se o projekt s názvem Genetika a epidemiologie mírné kognitivní poruchy. Sběr dat probíhal u praktických lékařů. Dotazníkové šetření obsahovalo Montrealský kognitivní test, informovaný souhlas a osobní dotazník. Zkoumaný soubor tvořilo 724 respondentů, z toho žen bylo 469 (64,8 %) a mužů 255 (35,2 %) ve věku od 20 do 92 let. Výsledky: V souboru bylo 231 (31,9 %) případů a 493 (68,1 %) kontrol. Mužů kontrol bylo 171 (67,1 %) a mužů případů 84 (32,9 %). Žen kontrol bylo 322 (68,7 %) a žen případů 147 (31,3 %). Případy nejčastěji chybovaly ve zrakově-prostorových úlohách (úloha č. 1 a 3), dále v úlohách zaměřených na pozornost (úloha č. 6A a 6C), na řečové schopnosti (úloha č. 7 a č. 8), abstrakci (úloha č. 9) a oddáleném vybavení (úloha č. 10). Rozdíly v úspěšnosti řešení těchto úloh mezi případy a kontrolami byly velké, neboť kontroly v těchto úlohách chybovaly méně než případy. Naopak malý rozdíl mezi případy a kontrolami v úspěšnosti řešení byl zaznamenán v úlohách, které se týkaly pojmenování (úloha č. 4), pozornosti (úloha č. 6B) a orientaci (úloha č. 11). Závěr: Ve všech úlohách byl zaznamenán statisticky významný rozdíl v počtu získaných bodů mezi případy a kontrolami. Mezi muži případy a muži kontrolami byl zjištěn statisticky významný rozdíl v počtu získaných bodů ve všech úlohách kromě úlohy č. 6B. Ve všech 11 úkolech byl zjištěn statisticky významný rozdíl mezi ženami případy a ženami kontrolami. Klíčovým výstupem bylo zjištění, že i tato práce dokazuje různé rozdíly ve schopnosti odpovídat na určité typy otázek dle osob hodnocených jako případy a kontroly.

Abstract:

Introduction: Mild cognitive impairment is a transient state between cognitive changes in aging and dementia. Normal daily activities are not disrupted. People with MCI usually have memory and concetration problems. Also, they have trouble processing new information and learning. However, all these symptoms do not mean dementia. These symptoms are mild. The diagnostic of Mild cognitive impairment is not based on certain symptoms. The changes in cognitive functions can be detected by neuropsychological tests. The Montreal Cognitive Test is a widely used screening assessment. Aim: The aim of this thesis was to analyze individual tasks in the Montreal Cognitive Test. The other purpose of this thesis was to find out in which tasks cases and controls made a lot of mistakes and compare the success rate of the cases and controls in solution of the tasks. Methodology: The data were obtained from the questionnaire survey in the study. It was a project Genetics and Epidemiology of Mild Cognitive Impairment. The data were collected in the general practitioner´s office. The questionnaire survey included the Montreal Cognitive Test, informed consent and personal questionnaire. In the research group, there were 724 respondents. There were 469 (64,8 %) women and 255 (35,2 %) men. The respondents were between 20 and 92 years old. Results: There were 231 (31,9 %) cases and 493 (68,1 %) controls. Male controls were 171 (67,1 %) and men cases 84 (32,9 %). Women controls were 322 (68,7 %) and women cases 147 (31,3 %). Cases often made mistakes in visual-spatial tasks (tasks n. 1 and n. 3), atteintion-focused tasks (task n. 6A and n. 6C) speech skills (task n. 7 and n. 8), abstraction (task n. 9) and memory (task n. 10). There were great differences in the success rate between cases and controls, because controls did not often make mistakes. The other way around, there was a small difference between cases and controls in the success rate of the solution in tasks naming (task n. 4), attention (task n. 6B) and orientation (task n. 11). Conclusion: There was a statistically significant difference in the number of scored points between cases and controls in all tasks. There was a statistically significant difference in the number of scored points between men cases and men controls in all tasks except task number 6B. There was a statistically significant difference between women cases and women controls in all 11 tasks. The results showed that this thesis also proved differences in the ability to respond to types of questions according to persons rated as cases and controls.
