Depression in the elderly with rheumatoid arthritis

Abstract:

Cíl: Cílem diplomové práce bylo zjistit, do jaké míry spolu souvisí přítomnost a míra deprese s omezením funkčních schopností, stupněm bolesti a stádiem onemocnění revmatoidní artritidiu (RA). Metodika: Realizovaný výzkum měl charakter kvantitativního šetření, který probíhal u seniorů s onemocněním revmatickou artritídou. Sběr dat byl proveden pomocí standardizovaného dotazníku HAQ (Health Assesment Questionare), zkoumajícího omezení funkčních schopností, dotazníku BDI-II (Beckova samoposuzovací škála deprese) a bolest pomocí vizuální analogové škály (VAS). Aktivitu onemocnění RA charakterizoval index DAS28. Výzkumný soubor tvořilo 228 respondentů navštěvujících revmatologickou ambulanci v Ostravě a Hlučíně. Sběr dat byl prováděn v období říjen-prosinec 2017. Výsledky: Byl zjištěn 46% výskyt depreseu pacientů s revmatickou artritídou, a to v kategoriích mírné, střední a těžké deprese. Zbylých 54 % respondentů z celkového souboru vykazovalo minimální příznaky deprese. Tato deprese je lineárně ovlivňována závislostí na omezením funkčních schopností pacientů. Čím vyšší funkční omezení schopností pacienta, tím vyšší byl prokázán výskyt deprese. S rostoucí intenzitou bolesti způsobenou základním onemocněním, také rostla míra deprese. Dále jsme nepotvrdili, že na výskytu deprese měla vliv aktivita revmatoidní artritidy. Závěr: Deprese se při onemocnění revmatickou artritidou vyskytuje ve vysokém zastoupení. Výskyt deprese také ovlivňuje stupeň omezení funkčních schopností a bolest spojená se základním onemocněním. Aktivita revmatoidní artritidy nemá vliv na stupeň projevů deprese.

Abstract:

Aim: The aim of this Master's Thesis was to examine relationships between the presence and degree of depression and limitations of functional abilities, the degree of pain perception and progression stage of rheumatoid arthritis (RA). Method: The study was designed as a quantitative research in elderly people with rheumatoid arthritis. The data collection was performed using a standardized Health Assessment Questionnaire focused on limitations of functional abilities, BDI-II questionnaire (Beck Depression Inventory-II) and pain perception through a visual analogue scale (VAS). The RA activity was characterized through the DAS28 index. The patient group was formed by 228 outpatients attending the outpatient clinics in Ostrava and Hlučín from October to December 2017 Results: 46 % of patients with RA suffered with depression (mild, moderate and severe depression). The remaining 54% of patients showed only minimum or no symptoms of depression. A linear correlation between the occurrence of depression and patients' functional limitations was observed. Similarly, the increasing pain intensity caused by RA also led to an increase in the degree of depression. We have however not confirmed any correlation between the RA activity and degree or rate of depression. Conclusions: Depression is a very common affliction in RA patients. The occurrence and degree of depression is affected by the degree of functional limitations and the pain associated with RA. RA activity has no effect on either the degree or rate of depression in elderly people.
