Determining of Protein S100 while Injury of Brain

Abstract:

Stanovení proteinu S 100B se stále více prosazuje v diagnostice poškození mozku různé etiologie. Jde o úrazy hlavy, hypoxické poškození CNS, cévní mozkové příhody, metastatické poškození, neurodegenerativní nemoci a infekce. Klinici stále častěji využívají protein S 100B při tzv. malých poraněních hlavy. V takových případech je často rtg vyšetření bez patologického nálezu, ale elevace hladiny S 100B upozorní klinika na (byť drobné) poškození tkáně mozku a vede k přijetí adekvátních terapeutických opatření. Stanovení se provádí v séru. Cílem práce bylo, po teoretické stránce, stručně utřídit základní informace o využití proteinu S 100B v klinické diagnostice. Po praktické stránce pak zjistit přítomnost proteinu S 100B ve vzorcích punktátů, které byly odeslány do laboratoře ke stanovení Beta-trace proteinu (BTP) pro prověření přítomnosti likvoru. Materiálem byly punktáty či sekrety vytékající z nosu, ucha nebo z drénu od pacientů s traumatickým úrazem hlavy, páteře nebo od pacientů, kteří podstoupili neurochirurgický zákrok. Vzorky byly sbírány od 2. 10. 2006 do 18. 6. 2007. Výsledky a závěr : Celkem jsme analyzovali vzorky od devadesáti pacientů. U 20 pacientů jsme zjistili pozitivní hladinu BTP a zároveň i S 100B. Negativní nález jak proteinu S 100B tak BTP byl 40 pacientů. 21 vzorků bylo pozitivních na koncentraci S 100B, ale negativní na BTP. U 9 pacientů jsme pozorovali pozitivitu BTP a negativitu S 100B. Z pohledu diagnózy byl celkově soubor značně heterogenní. Rovněž se nepodařilo zjistit bližší údaje o dalším osudu pacientů (po ošetření byli přeloženi do různých nemocnic regionu). Z tohoto důvodu uvádíme výsledky ve formě vybraných kazuistik.

Abstract:

Assessment of protein S 100B is still gaining ground in the diagnostics of brain damages of various etiologies. These are head injuries, hypoxical damages of the CNS, vascular encephalic happening, damage metastasis, neurodegenerative illnesses and infections. Clinicians often use protein S 100B for small head injuries. In those cases X-ray examination is without pathological diagnosis but the elevation of the S 100B level draws clinician`s attention to (though small) damage of brain tissue and it leads to adoption of adequate therapeutical measures. The assessment is carried out in serum. The aim of the work is, in theory, to briefly classify basic information about the use of protein S 100B in clinical diagnostics. In practice it tries to discover the presence of protein S 100B in the samples of punctures which were sent to the laboratory in order to assess Beta-trace of protein for checking the presence of cerebrospinal fluid. Materials used were punctures or secretion from nose, ear or from drains from the patients with traumatic head or spine injury or from the patients who underwent neurosurgical intervention. The samples were collected between October 2, 2006 and June 18, 2007. Results and conclusion: In total, we analysed samples from 90 patients. In 20 patients we discovered positive both BTP and S 100B level. Negative diagnosis of both protein S 100B and BTP was identified in 40 patients. 21 samples were positive in S 100B but negative in BTP. 9 patients had positive BTP and positive S 100B diagnosis. From the diagnostical point of view, the set was rather heterogenious. In addition, it was not possible to get information about the further state of the patients (they were transported to other hospitals in the region after their treatment). For this reason we present the results in the form of selected casuistics.
