The detection of opportunistic infections in hematooncological patients using molecular biological methods

Abstract:

Závažnou komplikací léčby pacientů s hematologickou malignitou je rozvoj infekcí způsobených oportunními mikroorganismy. Tyto infekce jsou u pacientů se sníženou obranyschopností charakteristické těžkým průběhem a mohou vést až k úmrtí pacienta. Hlavní původce lze rozdělit na mykotické, bakteriální, a virové, jejichž laboratorní diagnostika u pacientů s hematologickým onemocněním je předmětem této disertační práce. Pravidelné sledování výskytu řady patogenů se v současné době stalo součástí léčebných protokolů a posttransplantační péče hematoonkologických pacientů. Předpokladem pro přesnou detekci patogenů je využití vhodných laboratorních metod. V současné době jsou preferovány molekulárně genetické metody, které představují vysoce citlivý detekční systém. Cílem této disertační práce bylo rozšíření spektra vyšetření oportunních patogenů prováděných na našem pracovišti. Zavedli jsme detekci viru Epstein-Barrové a adenovirů v periferní krvi pomocí real-time PCR, která umožňuje sledovat dynamiku virové nálože v průběhu infekce. Dále jsme zavedli detekci širokého spektra respiračních virů zahrnujících respiračně syncytiální virus, parainfluenza virus 1, 2 a 3, adenoviry 1-7 a influenza virus A a B ve vzorcích tekutiny získané bronchoalveolární laváží a v nazofaryngeálních stěrech pomocí kvalitativních multiplex PCR metod a detekci lidského metapneumoviru pomocí real-time PCR. Vypracovali jsme laboratorní postupy pro rutinní detekci těchto patogenů a provedli jsme retrospektivní analýzy klinických vzorků pacientů s hematologickým onemocněním za účelem stanovení frekvence a závažnosti výskytu jednotlivých patogenů. Výsledkem korelace získaných dat a klinického stavu pacientů jsou doporučení pro frekvenci odběrů a testování konkrétních patogenů u pacientů Interní hematologické a onkologické kliniky FN Brno a LF MU. V oblasti cytomegalovirových infekcí jsme se soustředili na problém vzniku rezistence viru na antivirotika a zavedli jsme metodu polymorfismu délky restrikčních fragmentů a přímého sekvenování pro odhalení mutací vedoucích k rezistenci cytomegaloviru v genech UL97 a UL54. Následně jsme se blíže věnovali optimalizaci kvantitativních metod pro detekci případné koinfekce pacienta citlivou a mutovanou variantou CMV kmenů a zavedli jsme real-time PCR metody pro konkrétní mutace, které umožnili potvrdit výskyt obou variant viru a stanovit měnící se poměr mezi oběmi variantami viru v průběhu terapie CMV infekce.

Abstract:

The opportunistic infections represent serious complications in treatment of patients with hematologic malignancies. These infections are associated with severe course and high mortality in immunosuppressed individuals. The main causatives can be divided into three groups - fungal, bacterial and viral agens; the laboratory detection of viral pathogens is the subject of this work. The routine monitoring of many pathogens is a part of the treatment guidelines and post-transplant care of hematological patients. Therefore, the accurate laboratory detection of the pathogens is required. Currently, molecular genetic methods represent highly sensitive detection system. The main aim of present work was to enlarge the spectrum of opportunistic pathogen detection in our laboratory. We introduced quantification of Epstein-Barr virus and adenovirus in peripheral blood samples using real-time PCR method, which allows monitoring of viral load dynamic during infection. Then we introduced detection of wide spectrum of respiratory viruses including respiratory syncytial virus, parainfluenza virus 1, 2, 3, adenoviruses 1-7 and influenza virus A and B in bronchoalveolar lavage fluid samples and nasopharyngeal swab samples using qualitative multiplex PCR method and detection of human metapneumovirus using real-time PCR. We optimized laboratory processes to routine analysis of these pathogens and we performed the retrospective analysis of clinical samples of hematooncological patients. According to the correlation between clinical conditions and our laboratory results in patients we established the recommendations for sampling and testing frequency of specific pathogens in Department of Internal Medicine - Hematology and Oncology, University Hospital Brno. We introduced detection of mutations in two genes - UL97 and UL54, which are associated with development of CMV resistance to antivirotics. The mutations were identified using two methods - restriction fragment length polymorphism method and direct sequencing. Subsequently, we focused on optimisation of quantitative real-time PCR methods allowing the discrimination between mutant and wild type CMV strains. We performed three assays for confirmation the presence of both CMV strain variants and for determination of variable ratio between wild type and mutant CMV strains which depends on antiviral therapy.
