Propensity score: new dimension of mortality data analysis

Abstract:

V současné době jsou randomizované studie obecně považovány za zlatý standard pro stanovování účinku léčebných postupů. Bohužel, tyto studie nemusí vždy být proveditelné / etické nebo použitelné pro skutečné / životní podmínky. Proto jsme často nuceni spoléhat na observační studie; jejich výhoda v porovnání s randomizovanými klinickými studiemi je možnost zachytit skutečnou situaci na populační úrovni. Nicméně, získání spolehlivých výsledků z těchto studií vyžaduje sofistikované statistické metody v případech, kdy předpoklad náhodné distribuce vstupních faktorů do skupin pacientů není splněn; to může způsobit, že výsledek léčby není ovlivněn způsobem léčby, ale vstupními faktory. Jednou možnou metodou, jak upravit nenáhodné rozdělení ovlivňujících faktorů v nerandomizovaných klinických studiích, je analýza pomocí propensity skóre. Cílem této práce je přiblížit tuto relativně novou metodu a jako příklad jejího možného využití je v práci provedena její aplikace na data z českého registru srdečních selhání AHEAD. Na těchto datech byl sledován vliv paradoxu obezity a jeho vliv na hospitalizační mortalitu jako kofaktoru běžných rizikových faktorů. Výsledky ve vyváženém datovém souboru, získaného pomocí propensity skóre, se významně nelišily od původního celkového souboru. Použití propensity skóre pomohlo získat přesnější odhad léčebných výsledků a byl tak zde potvrzen paradox obezity.

Abstract:

Generally, randomized trials are currently considered as the gold standard to determine effect of a particular treatment. Unfortunately, these studies may not always be feasible / ethical or applicable in the real / living conditions. Therefore, we are often forced to rely on observational studies; their advantage in comparison with the randomized clinical trials is the ability to capture the real situation on a population level. However, obtaining reliable results from these studies requires sophisticated statistical methods, especially if the assumption of random distribution of input factors in patient groups is not met; such situation may cause that the outcome is affected by the input factors rather than the method of treatment. The analysis using propensity score is one of the available methods to modify the non-random distribution of affecting factors in randomized clinical trials. The aim of this work is to introduce this relatively new method and an example of its potential use is given in its subsequent application to data from the Czech registry of heart failure AHEAD. Effects of the obesity paradox and its impact on hospital mortality and risk factors was investigated in these datasets. Results from a balanced dataset, obtained using the propensity score, did not differ significantly from the original total dataset. Using the propensity score helped to obtain a more accurate estimate of the treatment effect and the obesity paradox was confirmed.
