Návrh diagnostického nástroje pro hodnocení orofaciální oblasti u dospělých

The dissertation deals with problems of diagnostics of muscle function of the orofacial region in adults with diagnosis of traumatic brain injury who have problems with the using of visual, auditory or verbal instructions relevant to the task due to coexisting cognitive deficit. The new diagnostic tool presented here assesses the particular orofacial regions not only from the functional point of view, but also from the point of view of cognitive deficits. This new diagnostic tool helps identify priority of speech targets due to the identified deficits and the muscle imbalances, including the identification of causes and connection with swallowing disorders and neurogenic speech disorders.

Abstract:

Disertační práce je věnována hodnocení orofaciální oblasti dospělých jedinců v klinické logopedii. Teoretická část se zabývá diagnózou kraniotraumatu, rehabilitací, vlastnostmi pohybové funkce a orofaciální oblastí. Empirická část práce zahrnuje vytvoření nového diagnostického nástroje - Orofaciálního profilu a jeho využití při vyšetření orofaciální oblasti u dospělých jedinců se získanou motorickou řečovou poruchou, kteří mají problémy s imitací. Pro splnění cíle byl proveden kvantitativní výzkum, vícefaktorový laboratorní experiment zabezpečený technikou jedné skupiny. Hlavním výstupem disertace je schéma vypracování rehabilitačního plánu dle skórovacích archů nového Orofaciálního profilu. Součástí je zachycení svalových dysbalancí, míry oslabení a dominance v celkovém obraze poruchy včetně zachycení kompenzačních a fixačních postojů a úrovně imitace.

Abstract:

The dissertation is devoted to the evaluation of the orofacial area of adults in clinical speech-language therapy. The theoretical part deals with the diagnosis of craniotrauma, rehabilitation, properties of motor function and orofacial area. The empirical part of the thesis includes the creation of a new diagnostic tool - the Orofacial Profile and its use in the examination of the orofacial area in adults with acquired motor speech disorder who have problems with imitation. To accomplish this goal, a quantitative research was performed, a multi-factor laboratory experiment secured by one group technique. The main outcome of the dissertation is a scheme for the elaboration of a rehabilitation plan according to the scoring sheets of the new Orofacial Profile. It includes assessment of muscle imbalances, levels of muscle weakness and dominance in the overall picture of the disorder. Attention is paid to capturing compensation and fixation mechanisms and the level of imitation.
