The effect of instrumented fusion after scoliosis surgery on adjacent unoperated spinal segments

Abstract:

Adolescentní idiopatická skolióza je nejčastější typ deformity páteře postihující děti ve věku od 10 do 18 let. Těžké deformity vyžadují chirurgickou léčbu. Zadní spinální instrumentovaná fúze je nejčastější chirurgickou volbou. Rozsah instrumentované fúze a zejména nejnižšího instrumentovaného obratle má přímý dopad na celkové držení těla pacienta a okolní volné pohybové páteřní segmenty. Tato disertační práce se zaměřuje na aspekty chirurgické léčby, jako je výběr instrumentovaného rozsahu, degenerace volných páteřních segmentů a celkovou posturu pacienta po operaci. V literatuře chybí konsenzus ohledně výběru LIV. V této práci je představena nová doplňková metoda pro selekci LIV s využitím úklonových snímků. Experimentální část se zabývá rozdílnými metodami fixace spodního instrumentovaného obratle a vlivu těchto metod na první volný disk pod instrumentací.

Abstract:

Adolescent idiopathic scoliosis is the most common type of scoliosis affecting children 10-18 years of age. Severe deformities require surgical treatment. Posterior spinal instrumented fusion is the most common surgical choice. The range of instrumented fusion and especially the lowest instrumented vertebra selection has a direct impact on the global posture of the patient and the rest of the unfused mobile spinal segments. This dissertation focuses on aspects of surgical treatment such as the selection of instrumented range, degeneration of unfused spinal segments, and compensated posture. There is a lack of consensus on the lowest instrumented vertebra selection in the literature. A novel supplementary method for the lowest instrumented vertebra selection with the use of bending films is presented in this work. The experimental part deals with different methods of fixation of the lowest instrumented vertebra and the effect of these methods on the first free disc below the instrumentation.
