Involvement of subthalamic nucleus in executive functions. Intracranial recording studies

Abstract:

Výsledky naší práce potvrzují, že bazální ganglia a zde především subtalamické jádro (STN) se nezúčasťnují pouze motorických, ale také kognitivních funkcií. Naše data z nahrávání evokovaných potenciálů z hlubokých elektrod v STN naznačují, že kognitivní role subtalamického jádra se liší od funkce jiných částí bazálních ganglií (striatum, pallidum), které již byli zkoumány. Na rozdíl od těchto struktur nebyl v STN generován potenciál P3 za použití standardního protokolu. Naopak, v STN byl generován P3 pouze při použití modifikovaného paradigmatu s dvojitou úlohou. V tomto případě právě úkol se zvýšenými požadavkami na exekutivní funkce vedl ke generování P3 přímo v STN. Tento výsledek může indikovat specifické, od obtížnosti úkolu závislé, zapojení STN do kognitivního zpracování informace. V další práci vedla repetitivní transkraniální magnetická stimulace (rTMS) oblasti dolního frotnálního kortexu (IFC) ke zkrácení latence vlny P3. Zkrácení latence vlny P3 odpovídá zvýšené rychlosti kognitivního zpracování. Naše práce potvrzuje, že IFC se podílí na organizaci kognitvních a především exekutivních funkcií v subtalamickém jádře. Kognitivní aktivity zahrnující STN jsou pravděpodobně organizovány prostřednictvím tzv. hyperpřímé dráhy spojující IFC a STN. Na tyto funkce pravděpodobně rozhodujícím způsobem nevplývá okruh kortex-bazální ganglia-thalamus. Naše výsledky podporují teorii rozsáhlé a komplexní neurokognitivní sítě, která se účastní exekutivních funkcií a potvrzují roli STN v kognitivním zpracování.

Abstract:

Our work shows that the basal ganglia and specifically the STN is involved not only in motor regulations. Our recordings of event-related potentials from the STN suggest a specific cognitive role of the STN that differs from the role of the BG nuclei investigated earlier. In contrast to the striatum and pallidum no standard oddball P3-like potential was generated in the STN. On the contrary, the modified protocol with a dual task elicited local field P3-like potentials within the STN. In this case, only the task with an increased demand on executive functions produced the P3-like local field ERPs. This can indicate a specific, task-related involvement of the STN in the cognitive processing of information. The shortening of P3 latency after modulation of the IFC by the rTMS corresponds to the increased speed of cognitive processing. Our finding that the latency change can be interpreted as an evidence that the IFC modifies also the cognitve and namely the executive processes in the STN. Cognitive functioning of the STN is possibly processed via a hyperdirect pathway and without the involvement of the cortico-basal ganglia – thalamocortical circuits. Overall, our results support the theory of a widespread and complex neurocoognitive network of the executive functions, and underlie the importance of the STN in higher cognitive processing
