Molecular mechanisms of solid tumor chemoresistance

Abstract:

Karcinomy tlustého střeva a konečníku jsou celosvětově nejčastějším nádorovým onemocněním gastrointestinálního traktu. Během roku 2020 přibylo téměř dva miliony nových případů této nemoci. Protože omezený průnik léčiv do nádoru a vystavení buněk subletálním koncentracím chemoterapie jsou častou komplikací léčby vedoucí ke vzniku chemorezistence, věnovali jsme se v rámci projektu vývoji pracovního postupu, který umožňuje kvantifikovat hladinu léčiva ve 3D modelech kolorektálních nádorů a jeho následnou kolokalizaci s oblastmi živých/apoptotických/proliferujících/dormantních buněk. Pro dosažení tohoto cíle byla vybraná kombinace zobrazovací hmotnostní spektrometrie s matricí asistované laserové desorpce/ionizace (MALDI MSI) a imunohistochemie. K ověření použitelnosti metodologie pro nádorovou biologii byl zvolen perifosin, orální protinádorové léčivo inhibující signální dráhu PI3K/Akt. Lokalizace průniku léčiva ve 3D modelech byla provedena MALDI MSI a fenotyp nádorových buněk byl posuzován pomocí imunohistochemie s fluorescenční detekcí. Výsledný signál byl detekován laserovým skenovacím konfokálním mikroskopem (LSCM). Aby bylo možné jednoznačně přiřadit pixely obrázku MALDI k pixelům souvisejícího obrázku z LSCM, provedli jsme koregistraci založenou na referenčních značkách ,,fiducials”. V koregistrovaných distribucích byly pak ekvidistantní křivky začínající na hranici sféroidu generovány softwarem, který jsme vyvinuli v úzké spolupráci s Fakultou informatiky Masarykovy univerzity. Analýza signálů MALDI MSI a LSCM v jednotlivých vrstvách poskytla informaci o jejich výsledném rozložení a umožnila posouzení korelačních koeficientů. S využitím tohoto nástroje jsme zjistili, že ve 3D modelech perifosin nepronikl hlouběji než 100-150 µm od periferie sféroidu o průměru 1 000 µm. Dále jsme dokázali, že se v oblastech uvnitř sféroidu, kam perifosin nepronikl, vyskytují stále životaschopné buňky. Zajímalo nás tedy, jak specifické mikroprostředí nádoru může ovlivňovat cytotoxicitu a penetraci perifosinu. Zvýšená exprese proteinů extracelulární matrix, její tuhost, gradient kyslíku a živin, a především acidóza a hypoxie jsou hlavními znaky nádorového mikroprostředí, které mohou ovlivnit účinnost terapie. Navíc bylo zjištěno, že nádory tlustého střeva jsou vystaveny celé škále intraluminálního pH (5.9-9.2) s ohledem na to, ve které části střevního traktu se nádor nachází. Experimenty byly proto provedeny (i) s buňkami v monovrstvě, které byly vystaveny kyselině mléčné, hypoxii a kombinaci obou a (ii) s 3D modely nádorové tkáně (sféroidy), které přirozeně lépe mimikují skutečné mikroprostředí nádoru. Naše experimenty prokázaly, že acidóza a acidóza kombinovaná s hypoxií eliminují cytotoxicitu perifosinu jak ve 2D, tak ve 3D modelech nádorové tkáně. Navíc, v monovrstvách vystavených acidóze a acidóze kombinované s hypoxií byla detekována nižší intracelulární hladina perifosinu než při normálním pH. Pro zlepšení účinnosti perifosinu jsme navrhli dvě strategie, alkalizaci nádorového mikroprostředí a kombinovanou terapii s inhibitorem proteinové rodiny Bcl-2, ABT-737. Kombinace perifosinu a ABT-737 působila synergicky ve všech testovaných prostředích (neutrální pH/acidóza, hypoxie/normoxie). Závěrem lze říci, že se nám podařilo ověřit možné použití nově vyvinutého polo-automatického nástroje pro hodnocení účinnosti chemoterapie v základním výzkumu. Naše zjištění dále ukázala, že změna vnějšího pH nádoru silně moduluje cytotoxicitu perifosinu vůči nádorům tlustého střeva, ale bez ohledu na to je perifosin vždy účinnější v kombinaci s ABT-737. Abstract:

Colorectal cancer is the most common type of gastrointestinal cancer worldwide, with almost 2 million new cases in 2020. Limited drug penetration into the tumor and exposure of cells to chemotherapy at sublethal concentrations often stimulate chemoresistance development, which complicates the treatment. Therefore, in the presented work, we aimed to develop a workflow to quantify drugs level in 3D colorectal cancer models and to co-localize drug penetration with the regions of viable/apoptotic/proliferative/quiescent cells. A combination of matrix-assisted laser desorption/ionization mass spectrometry imaging (MALDI MSI) and immunohistochemistry methods was selected and optimized to achieve this aim. Perifosine, an oral anti-cancer drug, and inhibitor of the PI3K/Akt pathway were used as proof of concept to demonstrate the applicability of our approach. MALDI MSI of spheroid sections provided information about perifosine penetration into 3D tumor models and the cancer cell phenotypes were detected by immunohistochemistry with subsequent laser scanning confocal microscopy (LSCM). To uniquely assign the pixels of the MALDI image to those of the related LSCM image, coregistration based on fiducials was carried out. In coregistered signal distributions, equidistant curves starting at the spheroid boundary were generated by software developed in close collaborations with the Faculty of Informatics, Masaryk University. Counting the MALDI MSI and LSCM signals along the curves yielded the signal's distribution and allowed the assessment of correlation coefficients. Using this tool, we have found that perifosine did not penetrate deeper than 100-150 µm from the periphery of the spheroid with a diameter of 1 000 µm. We also proved that cells were still viable in the areas inside the spheroid where perifosine was not able to penetrate. Therefore, we were interested in how the specific tumor microenvironment affects the cytotoxicity and penetration of perifosine. Increased expression of extracellular matrix proteins, increased rigidity, the gradient of oxygen and nutrients, and especially severe acidosis and hypoxia are hallmarks of tumor environment influencing drug efficacy in vivo. Moreover, colon tumors can be exposed to a large variety of intraluminal pH (5.9-9.2) depending on diet and their localization in the intestinal tract. Therefore, cytotoxicity tests were performed on (i) monolayers of cancer cells exposed to lactic acidosis, hypoxia, or a combination of these two factors and on (ii) 3D culture models (spheroids), which are supposed to simulate better the real tumor microenvironment. Our experiments showed that acidosis and acidosis combined with hypoxia diminished perifosine efficacy to both monolayers and spheroid CRC models. Corresponding to its diminished efficacy, the intracellular level of perifosine was lower in monolayers exposed to acidosis and acidosis combined with hypoxia compared to the normal pH. To enhance perifosine efficiency, we have proposed two strategies, alkalization of the tumor microenvironment and combination therapy with an inhibitor of the Bcl-2 protein family, ABT-737. The combination of perifosine and ABT-737 acted synergistically in all tested environments (neutral pH/acidosis, hypoxia/normoxia). In conclusion, we verified the application of newly developed semi-automatic software for anti-cancer drug evaluation in basic research. Additionally, our findings suggest that the alteration of the external tumor pH strongly modulates perifosine cytotoxicity to colorectal cancer but regardless of that, perifosine is always more efficient in combination with ABT-737. Thus, both tumor alkalization and combination therapy with ABT-737 could be considered as possible treatment strategies involving perifosine in colorectal cancer.
