The longitudinal observation of cognitive deficits in schizophrenia patients

Abstract:

Diplomová práce se zabývá sledováním kognitivního deficitu u pacientů se schizofrenií (N = 36) po dobu jednoho roku od jejich první epizody schizofrenie. Cílem empirické části bylo popsat profil a dynamiku kognitivního deficitu v oblastech exekutivních funkcí, paměti, pozornosti, psychomotorického tempa, vizuomotorické schopnosti a inteligence. Prokazovali jsme kognitivní deficit během první epizody schizofrenie, zda se pacienti po roce léčby zlepšili či zhoršili ve výše uvedených kognitivních funkcích, jestli se od sebe po roce lišili remitéři od neremitérů a zda inteligence ovlivňuje výsledek v použitých testech pro měření dílčích kognitivních funkcí. K výzkumu jsme užívali testové metody: Škála aktuální paměti, Bourdnova zkouška, Wisconsinský test třídění karet, Bentonův vizuálně retenční test, Ravenovy progresivní matrice, Test vizuálního a auditivního reakčního času a škálu pozitivních a negativních symptomů (PANSS). Pacienti byli měřeni při první schizofrenní epizodě a následně po jednom roce. Během prvního měření jsme u pacientů nalezli deficity v oblastech pozornosti, psychomotorického tempa, procesu vštěpování, exekutivních funkcích a ve zpracování vizuálních informací. Po roce léčby vykazovali pacienti zlepšení v oblasti celkové inteligence, pozornosti - její kvalitě a schopnosti koncentrace, rychleji a lépe zpracovávali vizuální informace, zrychlilo se jim celkové psychomotorické tempo. Nalezli jsme také, že remitéři se po roce zlepšili významně oproti neremitérům v oblasti celkové inteligence, pozornosti, konceptuální úrovni odpovědí (exekutivní funkce), vizuálním a auditivním reakčním čase. Poslední část prokázala pozitivní korelaci, testu inteligence s výsledky v testech: exekutivních funkcí, vizuomotorických schopností, auditivního a vizuálního reakčního času a pozornosti.

Abstract:

The thesis deals with the monitoring of cognitive deficits in schizophrenia patients (N = 36) for one year after their first schizophrenia episode. The aim of the empirical part was to describe the profile and dynamics of cognitive deficits in executive functions, memory, attention, psychomotor speed, visual- motoric ability and intelligence. We wanted to approve cognitive deficits during the first schizophrenia episode. Whether patients were improved or worsened in the cognitive functions after one year treatment. Whether were the differences between remitters and non-remitters in cognitive functions after one year. Whether intelligence affected the outcome of patients in tests used to measure cognitive functions. In this research part we used the test methods: Current Range of Memory (ŠAP) Bourdn's Test, Wisconsin Card Sorting Test, Benton's Test of Visual Retention, Raven's Progressive Matrices, Test of the visual and auditive reaction time and Positive and Negative Syndrome Scale (PANSS). Patients were measured at the first schizophrenic episode and then after one year. In the first measure of patients we found deficits in attention, psychomotor speed, in the process of inculcation, in executive functions and in the processing of visual information. After one year, patients have improved in general intelligence, in the attention's quality and in the ability to concentrate, in the faster and better processing visual information and they have had faster psychomotor speed. We also found that remitters improved significantly compared with non-remitters patients after a year in general intelligence, attention, conceptual level responses (executive function), auditive and visual reaction time. The last part of our research showed a positive correlation with intelligence test scores with tests focused on: executive functions, visual- motoric ability, auditive and visual reaction time and attention.
