The impact of gender on the course, treatment and prognosis of acute myocardial infarction

Abstract:

Dizertační práce se snaží identifikovat rozdíly v diagnostice, průběhu, léčbě a prognóze akutního infarktu myokardu (AIM) u mužů a žen a popsat jejich příčiny. Analyzovaný soubor čítá 889 pacientů s AIM, z nichž 35,5% tvoří ženy. Jako hlavní rozdíly byly zjištěny: u žen vyšší průměrný věk, častější výskyt hypertenze a diabetu, fibrilace síní a srdečního selhávání. Ačkoliv se způsob léčby AIM u žen a mužů neodlišoval, ženy měly během hospitalizace vyšší výskyt život ohrožujících komplikací. U žen byla pozorována vyšší mortalita, ať již v hospitalizační fázi AIM, tak během následného 3-letého sledování. Jako hlavní faktor ovlivňující hospitalizační mortalitu byl regresní analýzou určen výskyt významných komplikací, zatímco při hodnocení dlouhodobého přežití ovlivňoval prognózu nejvíce věk pacientů. Práce ukázala, že při AIM není pohlaví samostatným rizikovým faktorem mortality.

Abstract:

The aim of the doctoral thesis is the identification of differences between men and women with acute myocardial infarction (AMI). The attention is especially focused on the gender differences in the diagnostics, course, treatment and prognosis of AMI. The analysis includes 889 patients, 35,5% of them were the women. As the main differences were found: in women higher average age, from risk factors more frequently presented hypertension, diabetes, atrial fibrillation and heart failure. In women was also observed higher frequency of life-threating complications. Despite of the same treatment methods used in both men and women, the in-hospital mortality as well as the long-term survival remained poorer in women. In the regression analysis were found two principal factors responsible for higher women´s mortality: the in-hospital mortality was mainly influenced by the rate of significant complications, however the worse long-term prognosis was associated with higher age. The gender was not identified as the independent risk factor of mortality in AMI.
