The influence of orthodontic bracket base design on shear bond strength to enamel

Abstract:

Pevnost vazby ortodontického zámku ke sklovině je ovlivněna typem adhezivního materiálu k lepení zámků, morfologií a úpravou báze ortodontického zámku a přípravou skloviny před lepením. Měřili jsme pevnost vazby (SBS) a index zbytkového adheziva (ARI) nových a recyklovaných zámků. Vzorek tvořilo 5 kovových zámků z nerezavějící oceli s různou morfologií báze: Silver Star- mřížkovaná báze mikroleptaná, Omni Arch– vícevrstvá síťovaná báze SuperMesh ME s mikroleptáním, Discovery– laserem strukturovaná báze, Victory Series TM– jednoduchá síť s mikroleptáním a Sprint– báze s retenčními háčky s výřezy. Zámky byly nalepeny dvěma typy adheziv – kompozitní pryskyřicí Transbond XT a PM GIC Fuji Ortho na sklovinu extrahovaných premolárů (10 skupin po 15 vzorcích). Po odlepení byly zámky recyklovány pískováním Al2O3 – 90 µm. Pevnost vazby ke sklovině pro daná adheziva u nových i opískovaných zámků byla statisticky významně vyšší u kompozitního adheziva Transbond XT než u PM GIC Fuji Ortho. Kromě zámků Discovery, kde byly hodnoty SBS pro obě adheziva srovnatelné. Ve srovnání morfologie bází zámků a SBS bylo zřejmé, že statisticky nejvyšších hodnot SBS vykazovaly s oběma adhezivy zámky Victory Series (17,22 MPa) a Omni Arch (15,29MPa) tzn. síťované báze upravené mikroleptáním jedno resp. vícevrstvé, jak nové, tak i po opískování. Oproti tomu nejnižších hodnot SBS u nových i recyklovaných bází dosáhly zámky Discovery (7,98 MPa)-laserem strukturovaná báze. Ve srovnání SBS po opískování prokázaly recyklované zámky ve většině mírné zvýšení SBS, ale rozdíl nebyl statisticky významný, proto lze prohlásit, že pevnost vazby nových a pískovaných zámků byla rovnocenná. Pouze u zámků Victory došlo po opískování ke statisticky významnému snížení SBS (15,37 MPa). Morfologie báze testovaných nových zámků nevykázala statisticky významné rozdíly v hodnotách ARI. Pískováním zámků se hodnoty ARI obecně mírně zvýšily, nejvíce u zámků Discovery a Victory Series. Jako nejvýhodnější retenční povrch u námi testovaných kovových zámků se jeví mikroleptaná báze struktury sítě. Pískování je vhodnou metodou recyklace bází kovových zámků, která nevede ke statisticky významnému zhoršení retenčních vlastností bází kovových ortodontických zámků.

Abstract:

The bond strength between the orthodontic bracket and the enamel is influenced by the type of adhesive for bracket bonding, morphology and treatment of the bracket base and pre-treatment of the enamel prior to adhesion. This study aims at comparing the shear bond strength (SBS) and the ARI (Adhesive Remnant Index) value of five metal brackets with varied morphologies and base treatments, attached using two types of adhesive - Transbond XT composite resin and RM GIC Fuji Ortho. The study also attempted to follow changes in the SBS and ARI indices of these brackets after debonding and being recycled by sandblasting. The sample consisted of 5 metal brackets made of stainless steel with varying base morphology: Silver Star– microetched shaped bars base, Omni Arch– microetched SuperMesh ME multi-layered mesh base, Discovery – laser-structured base, Victory Series TM– microetched mesh base and Sprint– base with retention mushroom-shaped hooks. The brackets were bonded to the extracted premolar enamel (10 groups by 15 specimens) using two types of adhesives. After debonding, the brackets were recycled using Al2O3 – 90 µm sanding. The SBS for the adhesives used in new and recycled brackets was significantly higher statistically for the Transbond composite adhesive than for RM GIC Fuji Ortho. The only exception was the Discovery brackets in which the SBS values for both adhesives were comparable. Comparing the bracket base morphology and the SBS it is clear the highest SBS values statistically were shown when both adhesive types were used with Victory Series (17,22 MPa) brackets and Omni Arch (15,29 MPa) brackets, i.e., with meshed microetched bases employing single or multiple layers. This applies to both new and sanded brackets. The lowest SBS values for both new and recycled bases were reached using Discovery brackets (7,98 MPa) with a laser structured base. Comparing SBS values after sandblasting, the recycled brackets mostly showed a moderate increase in SBS, but the difference was not statistically significant. For this reason, it may be stated that the bond strength of new vs. sanded brackets was equivalent. Only in the case of the Victory brackets, did the SBS value decrease significantly statistically after sanding (15,37 MPa). The base morphology of the new brackets tested did not show statistically significant differences in ARI values. Sandblasting moderately increases ARI values, mostly when using Discovery and Victory Series brackets. The meshed microetched base seems to be the most convenient retention surface in the metal brackets tested in this study. Sandblasting is a suitable method for bracket base recycling which does not lead to poorer characteristics of the metal bracket bases.
