Targeted Surgical Therapy of the Obstructive Sleep Apnea

Abstract:

Disertační práce se zabývá chirurgickou léčbou obstrukční spánkové apnoe (OSA). V současnosti jsou doporučovány chirurgické výkony především u pacientů s lehkou OSA. Naší prací jsme chtěli prokázat, že jsme schopni dosáhnout srovnatelnou efektivitu chirurgické léčby pro všechny stupně OSA. Existují faktory, které ovlivňují efektivitu chirurgického výkonu, mezi ně patří lokalizace a míra obstrukce v horních dýchacích cestách (HDC), dále hodnota apnoe/hypopnoe indexu (AHI) a body mass indexu (BMI). Pro popis obstrukce v HDC využíváme v bdělém stavu Mallampatiho klasifikaci a Friedmanovu klasifikaci, dále endoskopii v léky navozeném spánku (DISE). Mezi negativní prognostické prediktory úspěšnosti chirurgické léčby OSA patří významný cirkulární uzávěr a předozadní uzávěr v oblasti měkkého patra, dále obstrukce v retrobazilinguální a supraglotické oblasti, vyšší hodnoty AHI a BMI nad 30. Mezi pozitivní faktory pak patří obstrukce v oblasti orofaryngu nad 50 %. Znalost těchto prediktorů nám umožňuje vytipovat pacienty vhodné k chirurgické léčbě. Chirurgická léčba OSA má při standardizované diagnostice místa obstrukce HDC a při odpovídajícímu výběru chirurgického výkonu velmi dobrou efektivitu, a to pro všechny stupně OSA.

Abstract:

The dissertation deals with the surgical treatment of obstructive sleep apnea (OSA). Currently, surgical procedures are recommended mainly for patients with mild OSA. With our study, we wanted to demonstrate that we are able to achieve comparable effectiveness of surgical treatment for all degrees of OSA. There are factors that influence the effectiveness of the surgical procedure, including the location and severity of obstruction in the upper airway, as well as the value of the apnea/hypopnea index (AHI) and body mass index (BMI). For descripton of upper airway obstruction, we use Mallampati's classification and Friedman's classification in the awake state, as well as drug-induced endoscopy (DISE). The negative prognostic predictors of successful surgical treatment of OSA include significant circular closure and anteroposterior closure in the area of the soft palate, as well as obstruction in the retrobasilingual and supraglottic areas, higher values of AHI and BMI above 30. The positive factors include more than fifty percent obstruction in the area of the oropharynx. Knowledge of these predictors allows us to select patients suitable for surgical treatment. The surgical treatment of OSA is very efficient in all degrees of severity of OSA provided the standardized diagnosis of the site of obstruction is made and the surgical procedure is selected accordingly.
