Physiotherapy in patients with whiplash syndrome

Abstract:

Úvod: Práce se zabývá fyzioterapií u pacienta s whiplash syndromem. Jedná se o soubor symptomů vznikajících nejčastěji vlivem automobilové nehody přenesením neadekvátní energie do krční páteře a hlavy, což vede k poranění kostěných struktur i měkkých tkání. To má za následek dlouhotrvající obtíže, např. bolesti krční páteře společně s omezením rozsahu pohybu, migrény a celkové zhoršení kvality života. Teoretická část je věnována anatomii a kineziologii krční páteře, zároveň je doplněna o etiopatogenezi poranění, klinický obraz a diagnostické postupy. V neposlední řadě jsou v teoretické části popsané způsoby léčby zahrnující komplexní fyzioterapii. Cíl práce: Cílem této práce je přispět k řešení problematiky fyzioterapie u pacienta s whiplash syndromem volbou vhodných metod a konceptů. Metodika: V této bakalářské práci je zpracována kazuistika 32letého pacienta s whiplash syndromem způsobeným automobilovou nehodou, která se udála v červenci roku 2022. V období od 17. 11. 2023 do 19. 1. 2024 probíhaly jednou týdně terapie u pacienta doma, kterých bylo celkově deset. Během první terapie byl proveden vstupní kineziologický rozbor a následně navržen krátkodobý plán fyzioterapie. Mezi nejstěžejnější metodiky a koncepty praktické části této práce patří terapie měkkých tkání, postizometrická relaxace, akrální koaktivační terapie, dynamická neuromuskulární stabilizace, postfacilitační strečink a relaxační techniky. V průběhu poslední terapie proběhlo výstupní vyšetření spolu se stanovením dlouhodobého fyzioterapeutického plánu. Výsledky: Došlo k výraznému snížení intenzity bolesti v oblasti krční páteře. Dále se zvýšil rozsah pohybu, a to zejména do rotace a lateroflexe, zároveň došlo k uvolnění hypertonických svalů a přilehlých měkkých tkání. Zlepšil se pohybový stereotyp flexe krku a aktivace hlubokého stabilizačního systému. Fyzioterapeutické intervence měly pozitivní vliv i na pacientův psychický stav. Závěr: Dle výsledků je zřejmé, že metody a koncepty, které byly zvoleny a obsaženy v krátkodobém fyzioterapeutickém plánu, měly dobrou účinnost na řešení dané problematiky.

Abstract:

Introduction: The bachelor thesis deals with physiotherapy in a patient with whiplash syndrome. It is a set of symptoms arising most commonly due to a car accident, when excessive energy is transferred to the cervical spine and head, resulting in injury to bone structures and soft tissues. This leads to long-lasting problems such as cervical spine pain along with restricted range of motion, migraines and a general deterioration in quality of life. The heoretical part is focused on the anatomy and kinesiology of the cervical spine, while the etiopathogenesis of the injury, clinical picture and diagnostic procedures are added. Furthermore, the theoretical part describes treatment methods including complex physiotherapy. Aim: The aim of the bachelor thesis is to contribute to the solution of physiotherapy problems in a patient with whiplash syndrome by choosing appropriate physiotherapy methods and concepts. Methods: In this bachelor thesis, a case report of a 32-year-old patient with whiplash syndrome caused by a car accident in July 2022 is presented. Between 17. 11. 2023 and 19. 1. 2024, once a week therapy sessions were held at the patient's home, with a total of ten sessions. During the first therapy session, an initial kinesiological analysis was completed and subsequently a short-term physiotherapy plan was designed. The most fundamental methodologies and concepts of the practical part of this thesis include soft tissue therapy, post isometric relaxation, acral coactivation therapy, dynamic neuromuscular stabilisation, post-facilitation stretch and relaxation techniques. During the last therapy, a final examination was performed to compare the results. Results: Following the prescribed physiotherapy methods in the short-term plan, the intensity of pain in the cervical spine was significantly reduced. Furthermore, the range of motion increased, especially into rotation and lateroflexion, while the hypertonic muscles and adjacent soft tissues became relaxed. The movement pattern of neck flexion and activation of the deep stabilization system improved. Physiotherapy interventions also had a positive effect on the patient's psychological condition. Conclusion: According to the results, it is clear that the methods and concepts that were chosen and included in the short-term physiotherapy plan had high effectiveness in solving the given problem.
