Molecular pathogenesis of agressive chronic lymphocytic leukemia

Abstract:

Cílem bakalářské práce bylo shrnout současné poznatky o molekulární patogenezi chronické lymfatické leukémie (CLL) s důrazem na její agresivní subtypy. CLL je nejčastější leukémií dospělého věku v České republice a vyznačuje se extrémně variabilním klinickým průběhem. U pacientů s indolentním onemocněním můžeme očekávat přežití až 25 let, zatímco u pacientů s agresivním onemocněním je jeho medián 2-3 roky. Přestože její celková patogeneze nebyla zatím uspokojivě vysvětlena, je zkoumáno několik oblastí směřujících k jejímu objasnění. Mezi hlavní oblasti zájmu patří poruchy apoptických drah, BCR signalizace, role mikroprostředí, vliv cytogenetických abnormalit a deregulace microRNA. Pochopení patogeneze agresivních subtypů je klíčové pro cílenou léčbu a zlepšení prognózy rizikových pacientů.

Abstract:

The aim of this bachelor thesis was to review the current knowledge on the molecular pathogenesis of chronic lymphocytic leukemia (CLL) and mainly its aggressive subtypes. CLL is the most common leukemia in adults in the western world and it is characterised by an extremely variable clinical course. The life expectancy in patiens with an indolent form of the disease is about 25 years while the median survival of patients with an aggressive subtype is less then 3 years. There is an ongoing interest in several mechanisms which could possibly explain the enigmatic pathogenesis of this disease. The fields of greatest concern are the deregulated apoptotic machinery, BCR signalling, the role of a microenvironment, the effect of cytogenetic abnormalities, and the deregulation of microRNAs. The understanding of pathogenesis of aggressive subtypes plays a key role in the development of new therapeutic strategies and subsequent improvement of CLL prognosis.
