Depression in pregnancy and postpartum: a network perspective

Abstract:

Tématem této práce je deprese u žen v těhotenství, po porodu a u žen mimo perinatální období. V úvodu jsou shrnuty současné poznatky, které se deprese v těhotenství a po porodu týkají, se zaměřením také na používané metody pro screening deprese a na obtížnost diagnostiky deprese v těchto obdobích. Představena je také tzv. perspektiva kauzálních systémů (causal system perspective), která začala v posledních letech převažovat jako paradigma v nahlížení na duševní poruchy obecně a depresi specificky, a síťové modely, které tuto perspektivu umožňují testovat analyticky. Práce sestává ze tří empirických studií. Zatímco první studie se věnuje psychometrickému ověření metody Patient Health Questionnaire-9 pro screening deprese u obecné populace, druhá a třetí již za využití síťových modelů mapují souvislosti mezi symptomy deprese u žen v a mimo perinatální období. Druhá studie zkoumá pomocí síťových modelů povahu somatických symptomů v síti depresivních symptomů u žen v těhotenství a po porodu a srovnává ji se sítí u žen mimo perinatální období. Třetí studie pak pomocí nově formulovaných cross-lagged panelových síťových modelů mapuje souvislosti depresivních symptomů napříč časem u žen po porodu a v počátcích rodičovství do tří let věku dítěte. Závěry ze všech tří studií jsou v závěru práce souhrnně diskutovány.

Abstract:

The topic of this thesis is depression in women during pregnancy, after childbirth, and in women outside the perinatal period. In the introduction, the current findings related to depression during pregnancy and after childbirth are summarized, with a focus also on the methods used for screening depression and on the difficulty of diagnosing depression in these periods. The so-called causal system perspective is also introduced, which in recent years has begun to prevail as a paradigm in looking at mental disorders in general and depression specifically, as well as network mod-els that allow this perspective to be tested analytically. The work consists of three empirical studies. While the first study deals with the psychometric validation of the Patient Health Questionnaire-9 method for screening depression in the general population, the second and third, using network models, map the connections be-tween symptoms of depression in women in and outside the perinatal period. The second study uses network models to investigate the nature of somatic symptoms in the network of depressive symptoms in pregnant and postpartum women. It com-pares it to the network of symptoms in women outside the perinatal period. The third study, using newly formulated cross-lagged panel network models, maps the connections of depressive symptoms across time in women after childbirth and in the beginnings of parenthood up to three years of age of the child. The conclusions from all three studies are summarized at the end of the thesis.
