Cefalea: due approcci riabilitativi a confronto

Headache: comparison between two rehabilitation approaches.

Abstract:

This experimental study aims at giving a more exhaustive understanding and on the other hand aims at treating a widespread pathology: the headache. As this is an experimental study, the objective is to compare 2 different therapies to treat patients that suffer from tension-type headache and migraine. The treatment aims at profiling a functional assestment and choosing the most effective cure to treat and heal the patient. The comparison that stands out is related to the following rehad methods: pompage, stretching and deep massage for one group and mobilizations for the other group. More precisely, the objective of the resarch is to find the most effective method to heal the patients that suffer from headache, even though such methods are so different from each other. To achieve this, it will apply the following evaluation criteria: VAS, number of splitting headaches, articular ROM and the flexion/rotation test (TRF) e it will monitor the patient for 5 to evaluate changes of the above mentioned parameters. Such criteria are considered to be subjective by nature, with the exception of the TRF and number of splitting headaches, which is most objective criteria to evaluate the patient recovery. To achieve this acknowledgement I gathered together the data concerning a number of patients that suffer from cefalea tendiva and migraine, aged between 20 and 60 years old, and I made a comparison based on the data I've been provided with. The total number of patients is 38 (22 women and 16 men) and I divided them in 2 groups, A (19 patients) and B (19 patients), based on a random pattern. The fisiotheraphy intervention occured once every 5 days and each session lasted 45 min. The research demonstrates that patients conditions improved, both from a pain symptomatology point of view as well as their quality of life. CEFALEE: DUE APPROCCI RIABILITATIVI A CONFRONTO 14 The most important data is the articular ROM of TRF. All patients improved the ROM but the statistics shows that there are no substantial differences between group A and group B (p=0,7085). Finally, with the same number of patients in the two groups and the same frequency and length of treatments, an increase in the majority of parameters was noticed. For these reasons, this must be considered a preliminary experimental project and represents the starting line for future study to go beyond the above mentioned limits.

studio, di natura sperimentale, č indirizzato alla comprensione e al trattamento di una patologia ampiamente diffusa: la cefalea. Essendo un lavoro di ricerca l?obiettivo č quello di porre a confronto due metodiche di trattamento da apportare a pazienti affetti da cefalea tensiva ed emicrania. L?intervento č finalizzato, dunque, nel compiere una valutazione funzionale del paziente e di trattarlo successivamente con la terapia piů corretta ed efficiente ai fini del suo pieno recupero. Il confronto che si pone in evidenza riguarda le seguenti metodiche riabilitative: pompage, stretching e massoterapia per un gruppo e le mobilizzazioni per l?altro gruppo. In particolare vogliamo valutare quale tra i due possa essere piů indicato come strumento di supporto nel lavoro di fronte a soggetti affetti da cefalea. Consapevole del fatto che siano due tecniche completamente differenti, lo scopo č quello di verificare quale delle due possa essere maggiormente d?aiuto nel riportare il paziente a migliorare il suo stile di vita molto spesso compromesso dalla disabiilitŕ che porta questa patologia. Per questo ci si affiderŕ ai seguenti criteri di valutazione: VAS, numero di attacchi, questionario e ROM articolare el test di rotazione in flessione (TRF) e si monitorerŕ il paziente per un cinque sedute nel valutare cambiamenti dei parametri considerati. Tali criteri sono propriamente di origine soggettiva, ad eccezione della valutazione del TRF e del numero di attacchi che č ciň che di piů oggettivo č in mio possesso per ricevere delle risposte attendibili sul recupero del paziente. Per affrontare questo lavoro č stato chiesto a degli studi di fisioterapia e ad un centro con il quale collaboro come tirocinante la possibilitŕ di svolgere trattamenti su pazienti affetti da cefalea tensiva ed emicrania e sono stati catalogati pazienti di etŕ compresa tra i 20 e i 60 anni. Il totale dei pazienti č 38 (22 donne e 16 uomini) e sono stati suddivisi in maniera CEFALEE: DUE APPROCCI RIABILITATIVI A CONFRONTO 12 casuale nel gruppo A (19 pazienti) e nel gruppo B (19 pazienti). L?intervento fisioterapico č stato suddiviso con una frequenza di ogni cinque giorni della durata di circa 45 minuti. Lo studio ha dimostrato che, a livello soggettivo, i pazienti hanno avuto dei notevoli benefici. Il miglioramento č stato sia della sintomatologia dolorosa sia della qualitŕ di vita. Il dato perň che č da considerare maggiormente č il ROM articolare del test di rotazione in flessione. I singoli pazienti hanno megliorato il range di movimento ma la statistica dimostra come non ci siano sostanziali differenze nei risultati tra i due gruppi (p=0,7085). In conclusione, a paritŕ di numero di soggetti appartenenti ai due gruppi di studio, di frequenza settimanale delle sedute di lavoro e di durata complessiva del trattamento riabilitativo, si č osservato un miglioramento della maggior parte dei parametri considerati. Per tali ragioni questo studio č da considerarsi uno studio pilota preliminare, e vuole essere punto di partenza per futuri lavori in grado di superare i limiti sopraesposti.
