Clostridium difficile in geriatric patients

Abstract:

Infekce způsobená bakterií Clostridium difficile se stává závažným problémem zejména u geriatrických pacientů, kteří jsou významnou rizikovou skupinou. Vzhledem k narůstajícímu významu infekce způsobené Clostridium difficile nejen v České republice ale i celosvětově patří tato problematika k tématům, na která musíme soustředit naši pozornost. Obecně se ví, že právě vyšší věk je rizikovým faktorem této infekce spolu se střevní dysmikrobií, způsobenou předchozí léčbou antibiotiky a podpořenou dalšími rizikovými faktory jako je imobilita střeva, porucha slizniční imunity gastrointestinálniho traktu či hospitalizace pacienta. V posledních letech dochází k epidemii infekcí Clostridium difficile s těžkým průběhem se stoupající mortalitou, přičemž příčinou je šíření nového vysoce virulentního kmene ribotyp 027/NAP1/B1, který je charakterizován s vyšší produkcí toxinů a větší odolností vůči antibiotikům. Cílem této práce je zhodnotit u seniorů výskyt rizikových faktorů, které vedou k onemocnění CDI a sestavit model, pomoci kterého můžeme určit prognózu nemocného z hlediska mortality a na základě tohoto odhadu optimalizovat léčbu nemocného.

Abstract:

The disease caused by the bacterium Clostridium difficile is becoming a serious problem predominantly in geriatric patients who are a significant risk group. With regard to the growing importance of the infection caused by Clostridium difficile not only in the Czech Republic but also worldwide, this issue is among the topics on which we need to focus our attention. It is generally known, that a rather high age is a risk factor for this infection alongside the intestinal dysmicrobia caused by the prior treatment with antibiotics and other risk factors such as intestinal immobility, mucosal immunity of the gastrointestinal tract disorder or hospitalization. In recent years there have been epidemics of Clostridium difficile infection with severe course with increasing mortality, wherein its cause is the spread of a new, highly virulent strain of ribotype 027 / NAP1 / B1 which is characterized in a higher production of toxins and greater resistance to antibiotics. The goal of this study is to evaluate the occurrence of risk factors in seniors which lead to the Clostridium difficile infection and build a model with which we can determine the patient's prognosis in terms of mortality and on the basis of the estimate to optimize the patient's treatment.
