Comparison of etiological agents of sepsis in surgical and internal patients

Abstract:

Septický stav patří k velmi závažným stavům ohrožujících život pacienta. Průnik mikroorganismů do krevního řečiště může vést k těžké sepsi až septickému šoku, který ve vysokém procentu i přes intenzivní léčbu bývá spojen s úmrtím. Závažnost stavu závisí na jedné straně na stupni patogenity a virulenci mikroorganismu, na druhé straně na imunitním stavu pacienta. Bakalářská práce je zaměřena na problematiku a diagnostiku sepse, která napomáhá odhalení původců septických stavů. Rychlá identifikace původců má mimořádný význam pro zahájení správné antimikrobiální léčby, stanovení citlivosti na antibiotika a upřesnění diagnózy. Výsledky bakalářské práce poukazují na fakt, že výskyt sepse může být úzce spjat s věkem a pohlavím. Za povšimnutí stálo, že i přes veškerá hygienická a preventivní opatření se stále častěji setkáváme se sepsí způsobenou gramnegativními bakteriemi produkujícími širokospektré betalaktamázy (ESBL).

Abstract:

Sepsis belongs to very serious states endangering patients´ lives. The penetration of microorganism into the bloodstream can lead to severe sepsis or even to septic shock which has high mortality rate, despite intensive treatment. Seriousness of the state depends on the one hand on the degree of pathogenity and virulency of microorganism, on the other hand on the patient´s immune status. The barchelor thesis is focused on the issue of sepsis as well as its diagnostic which contributes to detection of ethiological agens of septic statuses. Immediate identification of the ethiological agens has a a particular value for initiation of the right antimicrobial treatment, determination of antibiotic sensitivity and for diagnosis specification. The findings of this barchelor thesis indicate that the sepsis occurence can be closely related to age and gender. It is worth noticing that despite all hygienic and preventive precautions we encounter the sepsis caused by gramnegative bacteria producing extended spectrum β-lactamases even more often.
