Use of the ACT method in trimalleolar fracture

Abstract:

Byla zpracována kazuistika 26leté pacientky s trimalleolární frakturou hlezenního kloubu. Fyzioterapie probíhala v 10 ti terapiích po dobu 2 měsíců. Byla odebrána anamnéza a proveden vstupní kineziologický rozbor, který zahrnoval vyšetření aspekcí, antropometrii, goniometrii, vyšetření svalové síly, hybnosti, svalového tonu a chůze. Na základě vstupního vyšetření jsem stanovila krátkodobý rehabilitační plán s využitím metod fyzioterapie, které jsou vhodné po operaci trimalleolární zlomeniny - polohování, techniky měkkých tkání, kryoterapie, postfacilitační strečink, postizometrická relaxace, aktivní cvičení, proprioceptivní neuromuskulární facilitace, dynamická neuromuskulární stabilizace. Hlavním cílem bylo zařadit do léčby akrální koaktivační terapii (ACT), která využívá motorických vzorů ontogenetického vývoje a klade si za cíl dosáhnout co nejefektivnějšího držení těla pro dosažení správné funkce svalů a kloubů. V polovině terapie byl kontrolně přeměřen rozsah pohybu. Při poslední terapii byl proveden výstupní kineziologický rozbor a stanoven dlouhodobý kineziologický plán, včetně cvičební jednotky pro doma se zaměřením na ACT.

Abstract:

A case report of a 26 year old patient with a trimalleolar fracture of the ankle joint was prepared. Physiotherapy was performed in 10 therapies for 2 months. Anamnesis was taken and an initial kinesiological analysis was performed, which included examination of aspects, anthropometry, and goniometry, examination of muscle strength, momentum, muscle tone and walking. Based on the initial examination, I determined a short-term rehabilitation plan, using physiotherapy methods that are suitable after trimalleolar fracture surgery - positioning, soft tissue techniques, cryotherapy, post-fasciation stretching, post-isometric relaxation, active exercise, proprioceptive neuromuscular facilitation, dynamic neuromuscular stabilization. The main goal was to include ACT in the treatment, which uses motor patterns of ontogenetic development and aims to achieve the most effective posture to achieve proper muscle and joint function. In the middle of the therapy, the range of motion was re-measured. During the last therapy, a final kinesiological analysis was performed and a long-term kinesiological plan was established, including a home exercise unit focusing on ACT.
