Molecular genetic analysis of LDLR gene variants in patients with ischemic heart disease

Abstract:

Gen pro LDL receptor je spjat s familiární hypercholesterolémií (FH), která může ve svém důsledku vést k závažným onemocněním jako je zejména ischemická choroba (ICHS) srdeční a cévní mozkové příhody. V genu pro LDL receptor bylo identifikováno mnoho patogenních mutací, ale také velké množství polymorfismů (SNP), jejichž význam není zatím znám. Cílem této bakalářské práce bylo zaměřit se na námi vybrané SNP c.1060+7T>C a c.1060+10G>C v exonu 7 a c.2232A>G v exonu 15 genu LDLR a zjistit jejich možnou korelaci s ischemickou chorobou srdeční. K tomuto účelu byla vybrána skupina 100 osob s normolipidimickým profilem a získaná data byla porovnána s výsledky 100 pacientů s ischemickou chorobou srdeční, jež svým klinickým obrazem odpovídali familiární hypercholesterolémii. Metodický postup pro tuto studii byla analýza křivek tání pomocí FRET sond na přístroji Light Cycler480. Rozdíl ve výskytu variant mezi kontrolní normolipidemickou skupinou a skupinou pacientů s FH a ICHS byl statisticky zpracován pomocí Fisherova přesného testu, popř. s Bonferroniho korekcí na mnohonásobné porovnávání. Díky analýze získaných dat můžeme vyslovit závěr, že polymorfismus c.1060+7T>C nelze považovat za provázaný s FH a ICHS, jelikož ve všech skupinách byly distribuce totožné (100% homozygotů). Polymorfismus c.2232A>G nevykazoval signifikantní rozdíl mezi soubory ani v zastoupení alelického stavu tohoto polymorfismu (p=0,153), ani v zastoupení jednotlivých alel (p=0,125) a není tudíž v asociaci s FH a ICHS. U sekvenční změny c.1060+10G>C byla prokázána v souboru normolipidemických pacientů signifikantně nižší četnost (p=0,038) homozygotů (10%) než v souboru FH s ICHS pacientů (26%). Z toho vyplývá, že tento polymorfismus může souviset s rozvojem FH a případné ischemické choroby srdeční. Pro úplné potvrzení tohoto tvrzení by bylo nutno nálezy ověřit na větším souboru pacientů.

Abstract:

The gene for LDL receptor is associated with familiar hypercholesterolemia (FH) that may lead to the development of serious diseases such as ischemic heart disease (IHD) and stroke. Thousands of pathogenic mutations, but also many polymorphisms (SNP), whose sense is not known so far, were identified in the gene for LDL receptor. The aim of this batchelor thesis is to detect the c.1060+7T>C and c.1060+10G>C SNPs in exon 7 and c.2232A>G in exon 15 of gene LDLR and to determine their possible correlation with ischemic heart disease. For this purpose a group of 100 persons with normolipidemic profile has been chosen and aquired data were compared with the results of 100 patients suffering from ischemic heart disease corresponding to the diagnosis of familiar hypercholesterolemia. The melting curve analysis using FRET probes on the Light Cycler 480 instrument was used to detect the SNPs. The differences in the variant occurrence between the normolipidemic group and the group of patients with FH and ischemic heart disease was statistically compared using Fisher´s exact test, alternatively Bonfferoni´s correction for multiple comparing. Based on the aquired data, we can conclude, that the polymorphism c.1060+7T>C cannot be considered to be associated with FH and IHD, because the distribution was the same in both groups (100% homozygots). Polymorphism c.2232A>G didn´t show signifikant difference between the groups, nor in representation of the allelic status of this polymorphism (p=0,153), not even in the representation of individual allels (p=125) and is that´s why not in association with FH and IHD. The sequence variant c.1060+10G>C had a significantly lower frequency (p=0,038) of homozygotes (10%) in the group of normolipidemic patients compared to the group of FH and IHD patients. This polymorphism can thus be associated with development of FH and the risk of ischemic heart disease. However, further study is necessesary to confirm this conclusion.
